Abstract
서론
위염전증은 위 자체가 회전된 비교적 드문 질환이다. 위염전은 회전축에 따라
mesenteroaxial 혹은 organoaxial volvulus, 회전 정도에 따라 불완전 혹은 완전 염전(partial
vs complete), 선행 질환의 유무에 따라 특발성 혹은 이차성 염전(idiopathic vs secondary)
으로 분류된다. 위염전의 증상은 급성인 경우 급작스럽고 심한 위상복부 동통이나 흉통을
야기하고 만성의 경우 심와부 불쾌감, 흉골하 작열감, 복부 팽만감 등의 비특이적 증상을 보
이는 등 다양하게 나타나므로 진단을 종종 놓치는 수가 있다. 단순 흉부 촬영이나 복부 촬
영, 바륨 촬영 등으로 진단이 가능하며, 동반된 위 탈장 등의 복부내 이상을 확인하는 것이
필요하다. 치료를 위해서는 수술이 필요한데 최근 내시경적 정복술이 보고되고 있고 이로써
만족할 만한 결과를 얻고 있다. 본 예에서는 토혈을 주소로 내원하여 만성 위염전으로 진단
받고 위내시경으로 정복된 예로 국내 보고가 없는 바 이에 보고하는 바이다.

Gastric volvulus is a rare condition in which the stomach rotates upon itself. Gastric
volvulus can be classified according to axis of rotation (mesenteroaxial vs organoaxial),
extent of rotation (partial vs complete), and underlying cause (idiopathic vs secondary).
The symptoms of the patients with gastric volvulus varies from mild, nonspecific vague
abdominal discomfort to sudden, severe pain. The gastric volvulus can be diagnosed by
a combination of chest and abdominal plain film and UGI barium study. The treatment
of patients with gastric volvulus has classically been surgery for reduction of the
volvulus and correction of the underlying abdominal pathologic feature predisposing to
volvulus formation. In the recent literature, there have been several reports of endoscopic
correction of gastric volvulus. We recently experienced a case of chronic gastric
volvulus which was corrected by endoscopic reduction. Thus, we report the case with a
brief review of literatures.
