Abstract
서론
폐동맥 색전증은 폐동맥이 다른 혈관계에서 생성된 혈전 혹은 이물질에 의해 막히는 것을
의미한다. 이 중 많은 예가 주로 하지 심부정맥 혈전에 의해서 발생되며 이외 종양세포, 지
방, 공기 및 탈, 전분 등 이물질에 의해서도 발생된다. 종양에 의한 폐동맥 색전증은 1897년
Schmidt가 위암 환자에서 발생한 것을 처음으로 보고하였으며 이후로 폐암, 췌장암, 신세포
암, 난소암, 간암, 운모세포암 등 여러 종양에 의해 발생된 보고들이 있다. 이 중 원발성 간
세포암에 의한 폐동맥 색전증의 보고는 약 10예 정도이고 국내에서는 현재까지 한 예만 보
고되었다. 원발성 간세포암의 주 증상은 상부 복통, 체중 감소, 식욕 부진 및 전신 무력 등
으로, 호흡곤란을 주소로 내원하여 폐동맥 색전증이 확인된 경우는 매우 드물다. 이에 저자
들은 호흡곤란을 주소로 내원하여 원발성 간세포암에 의한 폐동맥 색전증이 확인된 1예를
경험하고 보고하는 바이다.

Pulmonary embolism is a life threatening disease that needs early and accurate
diagnosis and prompt management. We have experienced a case of hepatocellular
carinoma with pulmonary embolism and subsequent pulmonary infarction. The abdominal
CT showed multiple geographic low-density masses in the liver The masses were
extended to the inferior versa cava. The amount of alpha-fetoprotein was 1,150 ng/ml.
The chest CT revealed almost complete obstruction of left main pulmonary artery and
right inferior lobar artery by emboli and subsequent pulmonary infarction. The evidence
of deep vein thrombosis and any other risk factor of pulmonary embolism was not
found. He died of sudden cardiac arrest on the 12th day after admission.
