Abstract
서론
궤양성 대장염은 주로 대장의 점막 및 점막하의 염증을 특징으로 하는 원인 불명의 만성
질환으로 질병의 경과에 따라 장출혈, 협착, 천공 및 대장암 등의 국소적 합병증과 피부, 관
절, 간담도, 눈, 골격계 등에 장외 합병증이 나타날 수 있다. 이외에 나타나는 전신적인 합병
증 중에는 대사 장애와 과응고 기전(hypercoaguable state)에 의한 혈전증이 드물게 올 수
있으며, 염증성 장질환 환자에서 혈전증의 빈도는 1.2%에서 6.4%까지 다양하게 보고되고
있다. 혈전증은 심부 정맥, 폐 등에서 나타날 수 있으며, 드물게 장간막 또는 문맥에 혈전이
나타날 수 있고, 일단 혈전증이 발생하면 사망률이 높은 것으로 알려져 있다.
저자들은 6년간 궤양성 대장염을 앓고 있는 27세의 여자 환자에서 발생한 상장간막 정맥
및 간문맥 혈전증을 heparin을 사용하여 합병증 없이 치료한 경험을 하였기에 보고하는 바
이다.

Thromboembolism is a infrequent extraintestinal complication of inflammatory bowel
disease. Especially, portal vein and superior mesenteric vein thrombosis are rare
complications of ulcerative colitis and its mortality is invariably fatal. The mechanism of
thrombolic complications in inflammatory bowel disease remains enigmatic. Moreover, the
clarification of the hypercoagulable state remains elusive. This report describes the case
of 27-year-old-female with active ulcerative colitis. The patient revealed portal vein and
superior mesenteric vein thrombosis with ascites detected by abdominal ultrasound
examination and computed tomography and successfully treated with combined
anticoagulation therapy.
