Abstract
요약
목적 : CTAP 및 CTHA는 간내 소간세포암을 발견하는데 매우 예민한 검사 방법으로 간
내 종양과 혈관과의 상호관계, 정확한 간내 종양의 해부학적 위치 등 매우 긴요한 정보를
얻을 수 있다. 이에 저자 등은 CTAP, CTHA 및 Lipiodol CT가 간암 환자의 술전 검사로서
의 의의에 대하여 알아보고자 하였다.
대상 및 방법 : 저자 등은 1996년 4월부터 1997년 8월까지 삼성서울병원에서 실시하였던
CTAP, CTHA 및 Lipiodol CT 중, 나선식 CT와 혈관 조영술상 외과적 적출이 가능하였고
수술을 목적으로 시행하였으며 술전 조직 검사로 간암이 증명되었고 Lipiodol CT로 추가적
종양 결절을 확인할 수 있었던 82예를 대상으로 하였다. 양쪽 대퇴동맥을 천자한 후 상장간
막동맥과 총간동맥 혹은 고유간동맥 기시부에 카테터를 위치하고 환자를 CT 방으로 옮겨
조영제를 주입하여 CTAP부터 시행한 후 CTHA를 시행하였다.
결과 : 나선형 CT와 혈관조영술에서 105개의 결절을, CTAP 및 CTHA에서 138개의 결절
을 발견하여 28%인 23예에서 33개의 결절을 추가적으로 발견할 수 있었으며 12%인 10예에
서 외과적 치료 방침에서 비외과적인 치료 방침으로 치료 방향이 바뀌었다. 치료 방침이 바
뀐 10예 중 11%인 9예는 편측엽에서 양측엽으로 종양의 해부학적인 병변이 확대되었고 1%
인 1예에서는 편측성이지만 결절이 4개 이상으로 증가한 경우이다. 또한 나선식 CT에서 종
양의 크기가 클수록 CTAP와 CTHA에서 추가 종양 결절을 발견할 화률이 높았으며
(p=0.03), 나선식 CT에서의 결절 갯수에 따라 CTAP와 CTHA에서 추가로 결절을 발견할
확률은 통계학적으로 차이가 없었다(p=0.84). Lipiodol CT를 실시하였던 27예 중 6예(22%)
에서 CTAP 및 CTHA에서 각각 6개의 추가 결절을 발견하였고 3예(11%)에서는 간세포암
의 병변이 양측엽으로 확대되어 외과적 치료 방침이 비외과적인 치료 방침으로 바뀌었다.
결론 : CTAP, CTHA 및 Lipiodol CT는 불필요한 외과적 수술을 피할 수 있게 하여 주는
데 매우 유용하리라 사료되며 CTAP, CTHA 및 Lipiodol CT의 정확한 예민도 및 특이도를
알기 위하여서는 병리소견과 비교하는 전향적인 연구가 뒤따라야 하리라 생각된다.

Background/Aims : Computed tomography during arterial portography (CTAP),
computed nomography during hepatic arteriography (CTHA) and Lipiodol CT are highly
sensitive methods to detect intrahepatic tumor, Their sensitivity and accuracy are over
90% and 85%, respectively. We evaluated the usefulness of CTAP, CTHA and Lipiodol
CT as a preoperative examination.
Methods : For eighty two patients with hepatocellular carcinoma which were considered
as the resectable on spiral CT and angiogram, CTAP and CTHA were performed. The
additional nodules on CTAP and CTHA were confirmed by Lipiodol CT.
Results : CTAP and CTHA could find additional nodules in 28% of the patients. The
treatment plan was changed from surgical to nonsurgical method in 12% of the patients.
The larger tumor nodules were on the spiral CT, the more additional tumor nodules
were found on CTAP and CTHA (p=0.03). The probability of finding additional nodules
on CTAP and CTHA is not related with the counts of tumor nodules on the spiral CT
(p=0.84). CTAP and CTHA could find additional nodules in 6 cases of 27 cases who
underwent Lipiodol CT. The treatment plan was changed to nonsurgical treatment in 3
cases by the results of CTAP and CTHA.
Conclusions : We concluded that CTAP, CTHA and Lipiodol CT are useful in avoiding
unnecessary operation.
