Abstract
서론
임신과 관련되어 나타나는 간의 합병증은 간내 담즙울체, 급성 지방간, HELLP (hemolysis,
elevated liver enzymes, low platelets) 증후군 등이 있으며, 드물게 간내 출혈로 발생하는
간피막하 혈종이 있다. 주로 전자간증이나 HELLP 증후군에서 발병하는 것으로 보고되고
있는 간내 출혈은 대개 임신 후반기나 분만 전후로 나타나며 간파열과 동반하여 발생시 산
모 및 태아 사망률을 높이는 치명적인 합병증으로, 전세계적으로 약 150예가 문헌 보고되고
있으나 국내에는 아직 없는 실정이다. 증상은 임신 말기 혹은 분만 후에 상복부 또는 우상
복부 및 우측 옆구리의 동통을 나타내거나 갑작스런 저혈압 등의 징후를 나타내며, 심한 정
도에 따라 보존적 치료나 외과적 수술로 치료하는 것으로 되어 있다.
저자들은 전자간증의 과거력이 없는 환자에서 분만 후 발생한 우측 옆구리와 우측 어깨의
동통을 주소로 내원하여 조기에 간피막하 혈종(subcapsular hematoma of liver)으로 진단되
어 보존적인 치료로 완치된 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

Spontaneous hepatic hemorrhage is a rare complication of pregnancy usually associated
with severe preeclampsia or the HELLP (hemolysis, elevated liver enzymes, low
platelets) syndrome. It usually occurs during the latter part of pregnancy or the
postpartum period. Although it is a rare pregnancy related liver disease, it could be a
life-threatening condition, especially if it is found after rupture. The incidence is high
among the older and multiparous women, but the pathogenesis of this disease has not
been fully understood yet. We experienced a patient having a subcapsular hematoma
during the postpartum period that was not associated with preeclampsia. She began to
have right flank and shoulder pain two days after delivery Abdominal CT scan revealed
a 7×12×20 cm sized subcapsular hematoma without rupture. She was treated
conservatively with a percutaneous hepatic drainage. Nineteen days after the procedure,
she was discharged without any complication. Two months later, follow-up
ultrasonography showed only minimal amount of hematoma.
