목적 : 최근 H. pylori에 대한 상부위장관질환의 기전이 중요하게 대두되면서 H. pylori에 감
염된 소화성 궤양 환자가 증상이 없는 비감염된 환자에 비해 혈청 가스트린치가 증가한다고
알려져 있다. 따라서 본 연구는 H. pylori 감염, 상부위장관질환의 조직염증의 정도, 그리고
질환군에 따른 혈청 가스트린치의 변화 및 연관성을 알아보고자 하였다. 대상 및 방법 :
1999년 2월부터 1997년 7월까지 상부위장관증상이 있어 경상대학교병원에 방문한 환자들에
서 상부위장관 내시경검사를 시행한 결과 H. pylori 감염군 99명과 비감염군 35명을 대상으
로 조직학적 소견을 조사하였고, 125I radioimmunoassay를 이용하여 혈청 가
스트린치를 측정하였다. 결과 : H. pylori 감염군 99명과 비감염군 35명 사이의 혈청 가스트
린치는 통계학적으로 유의한 차이가 없었으며, 만성 위염과 위궤양의 질환군끼리 H. pylori
감염군과 비감염군 간의 혈청 가스트린치도 통계학적으로 유의한 차이가 없었다. H. pylori
감염군에서 각 질환군의 조직염증 정도에 따라 혈청 가스트린치를 비교하였을 때, 모든 대
상군에서 위저부의 만성 염증세포의 침윤이 심할수록(55.1±45.8 pg/mL, 22.9±15.0 pg/mL,
p<0.05), 그리고 상부위장관 내시경 소견상 만성 위염을 가진 군에서 위저부의 만성 염증세
포 침윤이 심할수록 혈청 가스트린치가 통계학적으로 유의하게 높았다(52.3±38.1 pg/mL,
18.8±15.9 pg.mL, p<0.05). H. pylori 비감염군에서 각질환군의 조직염증 정도에 따라 혈청
가스트린치를 비교하였을 때, 모든 대상군에서 위저부의 만성 염증세포 침윤 정도가 심할수
록 (66.4±48.8 pg/mL, 39.8±40.4 pg/mL, p<0.05), 활성도 정도가 심할수록 (124.3±58.7
pg/mL, 46.0±39.5 pg/mL, p<0.05), 또한 만성 위염 환자에서 위저부의 활성도 정도가 심할
수록 혈청 가스트린치가 통계학적으로 유의하게 높았다(110.4±0 pg/mL, 42.2±31.5 pg/ml,
p<0.05). 같은 정도의 조직염증 소견을 보인 H. pylori 감염군과 비감염군 간의 혈청 가스트
린치는 통계학적으로 유의한 차이가 없었다. 결론 : 혈청 가스트린치는 상부위장관질환의 종
류나 H. pylori 감염의 유무와 관계없이 위저부의 조직염증 정도가 심할수록 상승하므로, H.
pylori 감염과 고가스트린혈증과는 직접적인 상관관계가 없을 것으로 생각된다.

Background/Aims : It is well known that serum gastrin titers in patients infected with
Helicobacter pylori (H. Pylori) are much higher than those in uninfected asymptomatic
controls. The aim of this study was to investigate the relationship between serum
gastrin titers and histologic scorings according to disease entities in Koreans. Methods :
Ninty-nine patients infected with H. pylori and 35 uninfected patients were enrolled in
this study and their serum gastrin titers were measured by 125I
radioim-munoassay. The H. pylori infection was determined by rapid urease test and
histology. Results : No significant differences in serum gastrin titers were noted
between the 99 patients infected with H. pylori and the 35 uninfected patients. Serum
gastrin titers in the all patients infected with H. pylori were significantly higher in the
fundus of high chronic lymphocyte infiltration score than in the fundus of low chronic
lymphocyte infiltration score (55.1±45.8 pg/mL vs 22.9±15.0 pg/mL, p<0.05). In addition,
serum gastrin titers in the uninfected patients were significantly higher in the fundus of
high chronic lymphocyte infiltration score than in the fundus of low chronic lymphocyte
infiltration score. Conclusions : Serum gastrin titers were significantly high in high
histologic score group. However, they were not significantly increased by H. pylori
infection or gastroduodenal disease entities. Therefore, there is no direct correlation
between H. pylori infection and hypergastrinemia.
