원발성 답즙성 간경변증의 임상 및 병리학적 고찰

Abstract
목적 :원발성 담즙성 간경변증은 우리 나라에서
는 매우 드문 질환으로 지금까지 28 예가 산발적으
로 보고되어 있다. 이에 저자들이 경험한 11 예에 대
한 임상 및 병리조직학적 소견을 고찰하였다. 대상
및 방법 :항미토콘드리아 항체가 양성이고 간조직
검사에서 원발성 담즙성 간경변증에 합당한 소견을
보인 11 예를 대상으로 연령 및 성별 분포, 임상증상,
생화학적 검사, 자가면역 항체, 병리조직학적 소견
등을 후향적으로 분석하였다. 결과 :연령은 47.1 ±
9.5 세 (남자 35.5 ±0.5,여자 49.7 ±8.7 세 )였으며 남자
가 2 예 (18.2%),여자가 9 예 (81.8%)로 남녀 비는
1:4.5 였다. 첫 내원 동기는 우연히 발견된 간 기능
이상이 4 예 (36.4%)로 가장 많았고 만성 피로감 3 예
(27.3%),황달을 동반한 소양증 3 예 (27.3%),황달 1
예 (9.0%)순이었다. 생화학적 검사로는 alkaline
phosphatase 와 γ-GTP 는 모든 예에서 현저히 증가
되었으며 각각 996.5 ±449.0, 385.7 ±202.3 IU/L 였
다. AST 와 ALT 치는 각각 183.5 ±151.3, 166.9 ±
144.1 IU/L 였다. 혈청 총빌리루빈치는 72.7% (8/11)
에서 증가되었으며 평균치는 4.7 ±5.1 mg/dL 였고,
총콜레스테롤치는 36.4% (4/11)에서 증가되어 있었
다. IgM 치는 87.5% (7/8)에서 증가되었고 평균치는
581.0 ±250.6 mg/dL 였다. 항핵 항체는 57.1% (4/7),
항평활근 항체는 25.0% (2/8)에서 양성이었다. 내시
경적 역행성 담관조영술 또는 자기공명 담관조영술
은 36.4% (4/11)에서 간내담관이 가늘어진 양상을
보였다. 간생검 소견으로는 간내담관의 파괴 10 예
(90 9%),담소관의 증식 9 예 (81.8%),문맥역의 염증
세포 침윤 10 예 (90.9%),간 실질의 염증세포 침윤
5 예 (45.5%),격벽의 섬유화 6 예 (54.5%),간경변증 4
예 (36.4%)였다. Mayo clinic prognostic index for-mula
를 이용하여 생존율을 예측하였을 때 7 년 생존
가능성은 0.59-1.0 까지 비교적 양호한 예후를 보였
다. 결론 :저자들이 경험한 원발성 담즙성 간경변증
의 임상적 특징은 서구와 비슷하였으나 첫 내원 동
기는 서구와 달리 우연히 발견된 간기능 이상이 많
았고, 예후는 비교적 양호하였다.

Background/Aims: Primary biliary cirrhosis (PBC) is a rare disease in Korea and only 28 cases have been reported so far. We have experienced 11 cases of PBC and analyzed its clinical and histological features. Methods: We reviewed 11 patients
with
PBC
diagnosed by the presence of antimitochondrial antibody and pathologic findings of bile duct injury, retrospectively. Results: The mean age was 47.1±9.5 years (male 35.5±0.5, female 49.7±8.7 years) and the male to female ratio was 1:4.5 (male
2/11,
female 9/11). The most common initial presentation was incidentally detected by abnormal results of liver function tests (36.4%). The laboratory findings revealed cholestatic pattern with markedly elevated levels of alkaline phosphatase
(996.5±449.0
IU/L) and γ-GTP (385.7±202.3 IU/L) in all patients. Pathologic findings revealed: destruction of the intrahepatic bile ducts in 10 patients (90.9%), proliferation of bile ductules in 9 patients (81.8%), infiltration of inflammatory cellls in
portal
tracts in 10 patients (90.9%), infiltration of inflammatory cells in hepatic parenchyma in 5 patients (45.5%), septal fibrosis (scarring) in 6 patients (54.5%) and cirrhosis in 4 patients (36.4%). The 7-year-survival probability according to Mayo
Clinic
prognostic index formula is relatively good (0.59-1.0). Conclusions: The clinical characteristics of PBC are similar to those of western countries. However, the case in which initial presentation was incidentally detected by abnormal results of
liver
function tests was more common than in western countries.
