Abstract
목적: Mosaic-like pattern(MLP)은 문맥압 항진성 위병증의 기본 요소 중의 하나로서, 간경변 환자에서 상부위장관 출혈의 원인 중의 하나로 그 중요성이 증가하고 있다. 그럼에도 불구하고 문맥압 항진성 위병증에 관한 공통된 분류 방법이 없으며, 문맥압 항진성 위병증과 간질환의 중증도의 연관성에 관한 이해도 매우 빈약한 실정이다. 저자들은 MLP의 중증도 및 발생 위치와 간질환의 중증도와의 연관성을 알아보고자 하였다.
대상 및 방법: 139명의 강경변 환자를 대상으로 상부위장관 내시경검사를 하였으며, New Italian Endoscopy Club (NIEC)의 분류에 의거하여 문맥압 항진성 위병증의 기본 요소 중 MLP에 중점을 두고 관찰하였다. 저자들은 NICE의 분류에 추가로 MLP의 발생 위치를 위저부 및 상부체부, 위저부에서 중부체부까지, 하체부까지, 그리고 전정부까지 세분하여 관찰하였다.
간질환의 중증도는 Child-Pugh 분류에 의거하여 결정하였으며, MLP의 중증도 및 발생위치와 간질환의 중증도와의 관계를 비교 분석하였다.
결과: 139명의 간경변 환자를 대상으로 177예의 내시경검사를 시행하였으며 남자는 114명, 여자는 25명이었고, 평균 나이는 50.1±10.2세이었다. 간경변의 원인은 60명이 알코올이 원인이었고, 바이러스가 원인인 경우가 76명, 그외 3명은 하대정맥 폐쇄, 일차성 담즙성 간병변 및 원인 미상이었다. Child-Pugh 분류 A가 82예, B가 57예, C가 38예였다. 8예(4.5%)에서 MLP가 보이지 않았으며, MLP의 중증도로 보았을 때, 경증 124에(70.1%), 중등증 37예(20.9%), 중증이 8예(4.5%)였다. MLP를 발생 위치로 세분하였을 때, 위저부에서 상부체부까지 6예(3.6%), 중부체부까지 64예(38.1%), 하부체부까지 62예(36.9%), 전정부까지 28예(16.7%)였다. MLP의 중증도와 발생위치는 Child-Pugh 분류와 통계학적으로 의미 있는 연관성을 보였다(p=0.001/0.000). 또한 MLP의 중증도와 발생 위치 상호간에도 통계학적으로 의미있는 연관성을 보였다(p=0.000).
결론: MLP는 단독으로도 간기능의 중증도를 나타내 주는 중요한 지표가 될 수 있으며, MLP의 중증도뿐만 아니라 그 발생 위치까지도 간기능의 중증도와 뚜렷한 연관관계가 있음을 알 수 있었다. 또한 간기능이 저하될수록 MLP의 발생 위치는 위저부와 상부체부에서 진정부쪽으로 진행함을 알 수 있었다.
Background/Aims: Mosaic-like pattern (MLP), one of the elementary lesions of portal hypertensive gastropathy, is common in patients with liver cirrhosis. The aim of this study was to examine the association between the severity/location of MLP and severity of the liver disease.
Methods: We carried out 177 endoscopic examinations in 139 cirrhotic patients and examined the severity of MLP on the basis of the New Italian Endoscopy Club classification. Additionally, we classified the MLP according to its location. Then, the association between severity/location of MLP and Child-Pugh classification was investigated.
Results: Child class A, B, and C were observed in 82, 57, and 38 cases, respectively. Severity grades of MLP of
none, mild, moderate, and severe were observed in 4.5%, 70.1%, 21.5%, 4.5% of the 177 examinations, respectively. The location of MLP in the high body, mid-body, low body, and antrum was confirmed in 3.6%, 38.1%, 36.9%, and 16.7% of the cases, respectively. MLP was not observed in 4.8% of the cases. The severity and location of MLP revealed statistically significant correlation with the Child-Pugh classification (p=0.001/0.000, respectively). Moreover, the severity and location of MLP were also correlated each other (p=0.000).
Conclusion: Not only the severity but also the location of MLP has a strong correlation with the severity of liver cirrhosis. As the liver function deteriorates, the location of MLP tends to extend toward the
antrum from the high body.
