Abstract

¸ñÀû: ´Ù¾çÇÑ ÃéÀå ³¶¼ºÁ¾¾çÀÇ ¼ö¼ú Àü °¨º°Áø´ÜÀÇ °¡´É¼º°ú ¼ö¼úÀû Ä¡·á¿¡ ´ëÇØ ÀÓ»óÀû °íÂûÀ» ÇÏ°íÀÚ ÇÏ¿´´Ù.

´ë»ó ¹× ¹æ¹ý: 1991³â 3¿ùºÎÅÍ 2002³â 5¿ù±îÁö ÃéÀå ³¶¼ºÁ¾¾çÀ¸·Î ¼ö¼ú ÈÄ Á¶Á÷ÇÐÀûÀ¸·Î È®ÁøµÈ 159¿¹¸¦ ´ë»óÀ¸·Î °Ë»ç¼Ò°ß, ¼ö¼ú ¼ú½Ä, ¼ö¼ú ÈÄ ÇÕº´Áõ ¹× ¿¹ÈÄ µîÀ» ºÐ¼®ÇÏ¿´´Ù. °á°ú: 159¿¹ Áß Àå¾×¼º ³¶¼ºÁ¾ 37¿¹(23%), Á¡¾×¼º ³¶¼ºÁ¾¾ç 24¿¹(15%), °íÇ÷¼º À¯µÎ¼º »óÇÇÁ¾¾ç 38¿¹(24%), Ãé°ü³» À¯µÎ»ó Á¡¾×Á¾¾ç 60¿¹(38%)¿´´Ù. ÃßÀû ±â°£ÀÇ Æò±Õ 37.8°³¿ù (0.4°³¿ù-135.8°³¿ù)À̾úÀ¸¸ç, ÀÌ Áß 10¸í »ç¸ÁÇÏ¿´´Âµ¥ Á¡¾×¼º ³¶¼ºÁ¾¾ç 1¿¹(4.1%), Ãé°ü³» À¯µÎ»ó Á¡¾×Á¾¾ç 9¿¹(15%)À̾ú´Ù. Àüü 5³â »ýÁ¸À²Àº 90.8%À̾ú´Ù. °á·Ð: ÃéÀ島¼ºÁ¾¾ç Áß ÀϺΠÁúȯÀº ¾Ç¼ºÈ­ °¡´É¼ºÀÌ ÀÖ°í ƯÈ÷ Ãé°ü³» À¯µÎ»ó Á¡¾×Á¾¾çÀº ¾Ç¼ºÀÇ °æ¿ì »ç¸Á·üÀÌ ³ô¾Æ °¨º°Áø´ÜÀÌ Áß¿äÇÏ°í ¼ö¼ú ÈÄ ÇÕº´Áõ ¹× ÀÌȯÀ²ÀÌ Àû¾î Àû±ØÀûÀÎ ¼ö¼úÀû Ä¡·á°¡ ÇÊ¿äÇÏ´Ù°í »ý°¢µÈ´Ù.

Background/Aims: The aim of this study was to review the cystic neoplasms of the pancreas.

Methods: We retrospectively reviewed the clinical records of 159 patients with cystic neoplasms of the pancreas. The patients were treated surgically at the Asan Medical Center between March 1991 and May 2002.

Results: There were 37 serous cystadenomas (23%), 24 mucinous cystic neoplasms (15%), 38 solid and papillary epithelial neoplasms (24%), and 60 intraductal papillary mucinous tumors (38%). The most common symptom was abdominal pain. Ultrasonography and computed tomography were used for diagnosis in 63.6% and 71.6% of the patients, respectively. Regarding surgical treatments, pancreaticoduodenectomy was performed in 25 cases,
pylorus preserving pancreticoduodenectomy in 42 cases, total pancreatectomy in 11 cases, distal pancreatectomy in 60 cases, spleen preserving distal pancreatectomy in 4 cases, segmental pancreatectomy in 14 cases, and local excision in 3 cases. The overall perioperative complication rate was 12%. The 32 patients had malignacy on histologic examinations. The overall 5 year survival rate was 90.8%.

Conclusion: For the cystic neoplasms of the pancreas we recommend an active strategy of resection considering the high possibility of malignancy and the diagnostic difficulties. The resection has usually led to a low morbidity, low mortality, and good prognosis.
