Abstract

¸ñ Àû: À§½Äµµ ¿ª·ù´Â À§³»¿ë¹°ÀÌ ºÒ¼öÀÇÀûÀ¸·Î ½Äµµ·Î ³Ñ¾î°¡´Â °ÍÀ¸·Î, ½Å»ý¾Æ ¹× ¿µÀ¯¾Æ¿¡¼­ ÇϺνĵµ °ý¾à±ÙÀÇ ¹ß´ÞÀÌ ÇغÎÇÐÀûÀ¸·Î ¹Ì¼÷Çϱ⠶§¹®¿¡, ÇϺνĵµ °ý¾à±ÙÀÇ ¼öÃà·ÂÀÌ ³·¾Æ Àß ÀϾ´Ù. ½Äµµ´Â 15°³¿ùÀÌ µÇ¾î¾ß ±â´ÉÀÌ ¼º¼÷ÇÏ°Ô µÇ¹Ç·Î ±× µ¿¾È À§¹èÃâ½Ã°£À» ´ÜÃà½Ãų ¼ö ÀÖ´Ù¸é À§½Äµµ ¿ª·ù¸¦ °¨¼Ò½Ãų ¼ö ÀÖÀ» °ÍÀ̸ç, À§¹èÃâ½Ã°£Àº ¼öÀ¯ÀÇ Á¾·ù¿Í ÀÚ¼¼¿¡ µû¶ó ¿µÇâÀ» ¹ÞÀ» °ÍÀÌ´Ù.

¹æ ¹ý: ÀúÀÚ´Â ¼öÀ¯Á¾·ù¿¡ µû¸¥ À§½Äµµ ¿ª·ù ºóµµÀÇ Â÷À̸¦ ¾Ë¾Æº¸°íÀÚ 1996³â 8¿ùºÎÅÍ 1999³â 6¿ù±îÁö ¼øõÇâ´ëÇк´¿ø¿¡ ÀæÀº ¿Ã·Á³¿À¸·Î ³»¿øÇÑ 6°³¿ù ¹Ì¸¸ÀÇ ¿µ¾Æ 16¸í(¸ðÀ¯ ¼öÀ¯±º 7·Ê, ºÐÀ¯ ¼öÀ¯±º 9·Ê)À» ´ë»óÀ¸·Î À§½Äµµ ¿ª·ùÁõ¿¡ ´ëÇÑ °Ë»ç¸¦ ½ÃÇàÇÏ¿´´Ù. °¢ ¼öÀ¯±ºÀ» 24½Ã°£ ½Äµµ pH °Ë»ç¸¦ ÅëÇÏ¿© ¿ª·ù Ƚ¼ö, ¿ª·ùÀ², °¡Àå ±ä ¿ª·ù ½Ã°£, 5ºÐ ÀÌ»ó Áö¼ÓµÈ ¿ª·ù Ƚ¼ö, Á÷¸³À§½Ã °¡Àå ±ä ¿ª·ù ½Ã°£, ¾Ó¿ÍÀ§½Ã °¡Àå ±ä ¿ª·ù ½Ã°£ µîÀÇ 6°¡Áö ¿ª·ùÁö¼ö·Î ºñ±³ÇÏ¿´°í Åë°è󸮴 Mann- Whitney °ËÁ¤À» ÅëÇÏ¿© ºÐ¼®ÇÏ¿´´Ù.

°á °ú:
1) ¿¬±¸ ´ë»óÀº ¸ðÀ¯ ¼öÀ¯±º 7·Ê, ºÐÀ¯ ¼öÀ¯±º 9·Ê ÃÑ 16·Ê¿´À¸¸ç Æò±Õ ¿ù·ÉÀº ¸ðÀ¯ ¼öÀ¯±ºÀÌ 2.1°³¿ù, ºÐÀ¯¼öÀ¯±ºÀÌ 2.6°³¿ù·Î Â÷ÀÌ°¡ ¾ø¾ú´Ù.
2) ¿ª·ù Ƚ¼ö´Â ¸ðÀ¯ ¼öÀ¯±ºÀÌ 244 151ȸ/ÀÏ, ºÐÀ¯ ¼öÀ¯±ºÀÌ 275 155ȸ/ÀÏ·Î Â÷ÀÌ°¡ ¾ø¾ú´Ù.
3) ¿ª·ùÀ²Àº ¸ðÀ¯ ¼öÀ¯±ºÀÌ 14 15%, ºÐÀ¯ ¼öÀ¯±ºÀÌ 28 22%·Î ¸ðÀ¯¼öÀ¯±ºÀÌ ³·¾Ò´Ù.
4) °¡Àå ±ä ¿ª·ù ½Ã°£Àº ¸ðÀ¯ ¼öÀ¯±ºÀÌ 20 28ºÐ, ºÐÀ¯ ¼öÀ¯±ºÀÌ 58 66ºÐÀ¸·Î ºÐÀ¯ ¼öÀ¯±ºÀÌ ±æ¾ú´Ù.
5) 5ºÐ ÀÌ»ó Áö¼ÓµÈ ¿ª·ù Ƚ¼ö´Â °¢°¢ 5 6ȸ¿Í 9 3ȸ·Î Â÷ÀÌ°¡ ¾ø¾ú´Ù.
6) Á÷¸³À§½Ã °¡Àå ±ä ¿ª·ù ½Ã°£Àº °¢°¢ 10 8ºÐ°ú 40 47ºÐÀ¸·Î ¸ðÀ¯ ¼öÀ¯±ºÀÌ ÀǹÌÀÖ°Ô Âª¾Ò´Ù.
7) ¾Ó¿ÍÀ§½Ã °¡Àå ±ä ¿ª·ù ½Ã°£Àº °¢°¢ 18 29ºÐ°ú 52 66ºÐÀ¸·Î ¸ðÀ¯ ¼öÀ¯±ºÀÌ ÀǹÌÀÖ°Ô Âª¾Ò´Ù.

°á ·Ð: ÀÌ»óÀÇ °á°ú·Î º¸¾Æ ½ÄµµÀÇ ±â´ÉÀÌ ¹Ì¼÷ÇÏ°í À§ÀÇ ¿¬µ¿¿îµ¿ÀÌ ¾ÆÁ÷ ¹ß´Þ Áß¿¡ ÀÖ´Â ¿µ¾ÆµéÀº ¸ðÀ¯ ¼öÀ¯¸¦ ÇÏ´Â °ÍÀÌ À§½Äµµ ¿ª·ùÁõÀ» ÁÙÀÏ ¼ö ÀÖ´Â ¹æ¹ýÀ̸ç ÀÌ´Â ¸ðÀ¯ ¼öÀ¯ÀÇ ¶Ç ÇϳªÀÇ ÀåÁ¡À̶ó°í ÇÒ ¼ö ÀÖ°Ú´Ù.

Purpose: Gastroesophgeal reflux (GER) is defined as involuntary movement of gastric contents into esophagus. Relaxation of lower esophageal sphincter caused by immature anatomical development in newborn and young infants produces GER frequently. We wanted to know whether the frequency of GER is influenced by feeding types and position or not. We studied in 16 subjects according to feeding types (breast feeding group: BFG-7, formula feeding group: FFG-9) who admitted to the Soonchunhyang university hospital for recurrent regurgitation with 24 hr esophageal pH monitoring from August 1996 to July 1999.

Methods: We compared two groups by number of reflux episode, reflux rate, longest episode, numbers of episodes lasting £¾5 minutes, longest episode in upright position and longest episode in supine position. We used Mann-Whitney test for statistical analysis.

Results: 1) The subjects were 7 infants in BFG and 9 infants in FFG, 16 in total, and mean age was 2.1, and 2.6 months for BFG and FFG, respectively. 2) The reflux numbers were 244 151/day, 275 155/day for BFG and FFG, respectively. 3) The reflux rate was 14 15% for BFG and 28 22% for FFG. It was lower in BFG. 4) The longest episode was 20 28 minutes for BFG and 58 66 minutes for FFG. It was significantly longer in FFG. 5) The numbers of episodes lasting £¾5 minutes were 5 6 for BFG and 9 3 for FFG. 6) The longest episode in upright position was 10 8 minutes for BFG and 40 47 minutes for FFG. It was significantly shorter in BFG. 7) The longest episode in supine position was 18 29 minutes for BFG and 52 66 minutes for FFG. It was significantly shorter in BFG.

Conclusion: Breast feeding is strongly recommended to reduce the regurgitation in infancy. It is an another benefit of breast feeding.
