Abstract
목적: 장내 장중첩증 원인 바이러스의 침습에 의한 임파선 비대 및 장중첩증으로 인한 교액성 장폐색증은 혈중 비타민 C의 감소를 동반할 수 있으며, 또한 체내 비타민 C가 부족할 경우 장내 adenovirus 감염 및 침습을 유발할 수 있을 것이다. 본
연구에서는 장중첩증과 전혈과 혈장의 비타민 C 농도와의 관계에 대해 알아보고자 하였다.
방법: 1995년 5월부터 1998년 12월까지 경상대학병원 응급실로 내원하여 진단받았던 장중첩증 환아 57명과 장중첩증 연구군과 연령 분포가 동일한 진주시에 거주하는 건강한 소아 256명을 대조군으로 2,4-dinitrophenylhydrazine 방법을 이용하여 전혈과 혈장 비타민 C를 측정하였고 장중첩증 환아의 나이, 성별, 백혈구 수, 발열 유무, 발병에서 진단까지의 시간, 혈변의 유무와 수술 유무 등을 후향적으로 조사, 비교하였다.
결과: 장중첩증으로 내원한 환아들의 평균 나이는 1년 3개월이었으며 남아가 39명, 여아가 18명이었다. 장중첩증 환아의 전혈 비타민 C농도는 1.49±0.64 ㎎/㎗로 대조군의 2.18±0.49 ㎎/㎗에 비해 낮았고(P=0.0001), 혈장 비타민 C농도는 0.59±0.36 ㎎/㎗로 대조군의 1.47±0.56 ㎎/㎗ 보다 낮았다(P=0.0001). 장중첩증 환아군에서 전혈 비타민 C 농도 중앙값이 여아 1.56 ㎎/㎗, 남아 1.34 ㎎/㎗로, 남아가 여아에 비해 낮았다(P=0.003). 그러나 장중첩증 환아의 연령, 백혈구수, 발열, 발병에서 진단까지의 시간, 혈변과 수술의 유무에 따라서는 전혈 및 혈장 비타민 C의 농도는 차이가 없었다.
결론: 장중첩증이 있는 환아는 건강 대조군에 비해 전혈과 혈장 비타민 C의 농도가 낮았지만 이것이 원인인지 또는 결과인지는 본 연구로 알 수 없었다. 장중첩증과 비타민 C의 관계를 정확하게 알기 위해서는 전향적인 연구가 필요할 것으로 사료된다.
Purpose: Adenovirus infection with swollen Peyer patches in the ileum, known as one of the causes of intussusception, may cause vitamin C depletion in human body because vitamin C is a first line antioxidant. Or low vitamin C status in human body makes the man more susceptible to infection of adenovirus in the ileum with a markedly swollen lymph node. In this study, we tried to find out the relationship between pediatric intussusceptions and vitamin C concentrations of whole blood and plasma.
Methods: Whole blood and plasma vitamin C concentrations of fifty-seven patients with intussusceptions from May 1995 to December 1998 at the Gyeongsang National University Hospital were compared with those of 256 normal healthy children. Vitamin C was measured by the 2,4-dinitrophenylhydrazine methods.
Results: The average age of patients (male=39, female=18) with intussusceptions was fifteen months of age. Whole blood vitamin C concentrations of patients and healthy children were 1.49±0.64 ㎎/㎗, and 2.18±0.49 ㎎/㎗, respectively. Plasma vitamin C concentrations of patients and healthy children were 0.59±0.36 ㎎/㎗, 1.47±0.56 ㎎/㎗, respectively. But no differences in the vitamin C concentrations of whole blood and plasma according to age, degree of leukocytosis, fever, interval from onset, hematochezia, and need for operation were found.
Conclusion: Whole blood and plasma vitamin C concentrations of patients with intussusceptions were lower than those of healthy children (P=0.0001). Prospective studies are needed to elucidate whether these results were consequences or causes of intussusceptions.
