Abstract

¸ñÀû: ¼Ò¾Æ±â´Â ¾ËÄÚ¿ÃÀ̳ª ¾à¹° µî¿¡ ÀÇÇÑ ÀÚ±ØÀÌ °ÅÀÇ ¾ø´Â ½Ã±â·Î H. pylori °¨¿°ÀÇ ÀÚ¿¬ °æ°ú¿Í ´Ü±â°£ÀÇ ¿µÇâÀ» ¿¬±¸Çϱ⿡ ÀûÇÕÇÑ ½Ã±âÀÌ´Ù. ÃÖ±Ù H. pylori °¨¿°ÀÇ ±âÀüÀ¸·Î Áß¿ä½ÃµÇ°í ÀÖ´Â À§ »óÇǼ¼Æ÷ Áõ½Ä°ú ¼¼Æ÷»ç¿¡ ´ëÇØ ¼Ò¾Æ¿¡¼­ ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.

¹æ¹ý: 1996³â 5¿ùºÎÅÍ 2001³â 6¿ù±îÁö ÀÌÈ­¿©ÀÚ´ëÇб³ ¸ñµ¿º´¿ø ¼Ò¾Æ°ú¿¡¼­ ¼ÒÈ­±â Áõ»óÀ¸·Î ³»½Ã°æÀ» ½ÃÇàÇÏ¿© H. pylori °¨¿°À¸·Î Áø´ÜµÈ 58¿¹¿Í °¨¿° À½¼º 40¿¹¸¦ ´ë»óÀ¸·Î ÇÏ¿´´Ù. H. pylori °¨¿° ¾ç¼ºÀº Á¶Á÷ÇÐÀûÀ¸·Î H. pylori ±ÕÀÌ °üÂûµÇ°í, CLO °Ë»ç¿Í ureC PCRÀÌ ÀüºÎ ¾ç¼ºÀÎ °æ¿ì·Î ÇÏ¿´´Ù. À§»ý°Ë Á¶Á÷¿¡¼­ °³Á¤µÈ ½Ãµå´Ï ü°è¸¦ ÀÌ¿ëÇÏ¿© Á¶Á÷ ¼Ò°ßÀ» ºÐ¼®ÇÏ°í, proliferating cell nuclear antigen (PCNA) ¹ßÇöÀ¸·Î À§ »óÇǼ¼Æ÷ Áõ½ÄÀÇ Á¤µµ¸¦, in situ terminal deoxynucleotidyl transferase- mediated d UTPnick-end labeling (TUNEL) ¹æ¹ýÀ¸·Î ¼¼Æ÷»çÀÇ Á¤µµ¸¦ Á¶»çÇÏ¿´´Ù.

°á°ú: 1) H. pylori °¨¿° ¾ç¼º¿¡¼­ ´ÙÇÙÇü Áß¼º±¸ÀÇ È°µ¿¼º(P=0.000), ¸¸¼º ¿°Áõ(P=0.000), »óÇǼջó(P=0.000), ¸²ÇÁ¿©Æ÷(P=0.000)ÀÇ Á¤µµ°¡ °¨¿° À½¼º¿¡ ºñÇÏ¿© À¯ÀÇÇÏ°Ô ³ô¾Ò´Ù. H. pylori °¨¿°¾ç¼º¿¡¼­ ÀåÇüÈ­»ýÀº °üÂûµÇÁö ¾Ê¾Ò´Ù. 2) H. pylori °¨¿° ¾ç¼º¿¡¼­ ¼¼Æ÷ Áõ½Ä ÁöÇ¥´Â 67.8¡¾18.13À¸·Î, À½¼º 54.8¡¾14.46¿¡ ºñÇÏ¿© À¯ÀÇÇÏ°Ô ³ô¾Ò´Ù(P=0.000). ¼¼Æ÷ Áõ½Ä ÁöÇ¥´Â H. pylori ¹Ðµµ°¡ Áõ°¡ÇÒ¼ö·Ï(r=0.277, P=0.007), ´ÙÇÙÇü Áß¼º±¸ÀÇ È°µ¿¼ºÀÌ Áõ°¡ÇÒ¼ö·Ï(r=0.280, P=0.007), ¸¸¼º ¿°ÁõÀÌ Áõ°¡ÇÒ¼ö·Ï(r=0.284, P=0.006) Áõ°¡ÇÏ¿´´Ù. 3) ¼¼Æ÷»ç ÁöÇ¥´Â H. pylori °¨¿° ¾ç¼º¿¡¼­ 0.44¡¾0.447, À½¼º¿¡¼­ 0.14¡¾0.196À¸·Î °¨¿° ¾ç¼º¿¡¼­ À½¼ºº¸´Ù À¯ÀÇÇÏ°Ô ³ô¾Ò´Ù(P=0.000). ¼¼Æ÷»ç ÁöÇ¥´Â H. pylori ¹Ðµµ°¡ Áõ°¡ÇÒ¼ö·Ï(r=0.472, P=0.000), ´ÙÇÙÇü Áß¼º±¸ÀÇ È°µ¿¼ºÀÌ Áõ°¡ÇÒ¼ö·Ï(r=0.370, P=0.001), ¸¸¼º ¿°ÁõÀÌ Áõ°¡ÇÒ¼ö·Ï(r=0.483, P=0.000) Áõ°¡ÇÏ¿´´Ù. 4) ¼¼Æ÷ Áõ½Ä ÁöÇ¥°¡ Áõ°¡ÇÒ¼ö·Ï ¼¼Æ÷»ç ÁöÇ¥´Â À¯ÀÇÇÏ°Ô Áõ°¡ÇÏ¿´´Ù(r=0.353, P=0.003).

°á·Ð: H. pylori °¨¿° ¼Ò¾Æ¿¡¼­ ¼¼Æ÷ Áõ½Ä ÁöÇ¥¿Í ¼¼Æ÷»ç ÁöÇ¥°¡ À¯ÀÇÇÏ°Ô Áõ°¡ÇÏ¿´À¸¸ç »ó°ü¼ºµµ À¯ÀÇÇÏ¿´´Ù. ÀÌ´Â ¼Ò¾Æ¿¡¼­ À§ »óÇǼ¼Æ÷ Áõ½Ä°ú ¼¼Æ÷»ç°¡ H. pyloriÀÇ º´Àο¡ Áß¿äÇÔÀ» ½Ã»çÇϸç, ¾ÕÀ¸·Î ¼¼Æ÷ Áõ½Ä°ú ¼¼Æ÷»çÀÇ ±âÀü, À¯¹ß ¿äÀÎ ¿Ü¿¡ ´Ù¸¥ º´µ¶ ÀÎÀÚ¿ÍÀÇ °ü·Ã¼º¿¡ ´ëÇÑ ¿¬±¸°¡ ÇÊ¿äÇϸ®¶ó »ý°¢µÈ´Ù.

Purpose: Dysregulation of gastric epithelial cell proliferation and apoptosis are important in development of ulcer, atrophy and neoplasia in Helicobacter pylori (H. pylori) infection. The aim of this study was to investigate the effect of
infection of H. pylori on gastric epithelial cell proliferation and apoptosis in children.

Methods: Histological grading by updated Sydney system, PCNA immunostaining and TUNEL method were performed in H. pylori positive (N=58) and negative (N=40) gastric biopsy specimens.

Results: In H. pylori positive children, there were significantly higher grade of polymorphonuclear neutrophil activity (P=0.000), chronic inflammation (P=0.000), epithelial damage (P=0.000) and lymphoid follicles (P=0.000) than in H. pylori negative children. Intestinal metaplasia was not seen in H. pylori positive children. PCNA index was significantly different between H. pylori positive children (67.8¡¾18.13) and H. pylori negative children (54.8¡¾14.46, P=0.000). There was positive correlation between PCNA index and H. pylori density (r=0.277, P=0.007), polymorphonuclear neutrophil activity (r=0.280, P=0.007) and chronic inflammation (r=0.284, P=0.006). Apoptosis index of H. pylori positive children (0.44¡¾0.447) was significantly higher than of H. pylori negative children (0.14¡¾0.196, P=0.000). There was positive correlation between apoptosis index and H. pylori density (r=0.472, P=0.000), polymorphonuclear neutrophil activity (r=0.370,
P=0.001) and chronic inflammation (r=0.483, P=0.000). There was positive correlation between PCNA index and apoptosis index (r=0.353, P=0.003).

Conclusion: The PCNA and apoptosis index in H. pylori positive children were significantly higher than in H. pylori negative children. This study suggested that gastric epithelial cell proliferation and apoptosis are important to pathogenesis of H. pylori infection in children.
