Abstract

¹æ ¹ý: ¿¬±¸´ë»óÀº ¹Ì¼÷¾Æ¸¦ Á¦¿ÜÇÑ À¯¼Ò¾Æ·Î ÃÑÁ¤¸Æ¿µ¾ç¹ý¿¡ ÀÇÇÑ ´ãÁó¿ïü ¹× °£È¿¼ÒÄ¡ÀÇ Áõ°¡¸¦ º¸ÀÌ°í Ç÷¿ªÇÐÀû, ´ë»çÀûÀ¸·Î ¾ÈÁ¤µÇ¾úÀ¸¸ç Áֱ⼺ Á¤¸Æ¿µ¾ç¹ýÀÌ °£±â´É ÀÌ»óÀ» º¸ÀÌ´Â Ãʱ⿡ ½ÃµµµÇ¾ú´ø 12·ÊÀÇ È¯¾ÆµéÀ̾ú´Ù. °á °ú: 1) ȯ¾ÆµéÀÇ Á¤Áß¿¬·É(median age)Àº Áֱ⼺ Á¤¸Æ¿µ¾ç¹ý ½ÃÀ۽à 4¼¼(»ýÈÄ 2°³¿ù¿¡¼­ 17¼¼)¿´°í ±âÀúÁúȯÀº ¸¸¼º°¡¼ºÀåÆó»öÁõ 4·Ê¸¦ ºñ·ÔÇÏ¿© ´Ù¾çÇÏ¿´´Ù. 2) µ¿ÀÏ ¿¹¿¡¼­´Â ´ëºÎºÐ¿¡¼­ Åõ¿©µÈ ´Ü¹é°ú Æ÷µµ´ç, Áö¹æÀÇ Åõ¿©·®¿¡ Â÷ÀÌ°¡ ¾ø¾ú´Ù(p£¼0.05). Áֱ⼺ Á¤¸Æ¿µ¾ç¹ýÀ» ½ÃÇàÇÏ´Â µ¿¾È 1·Ê¿¡¼­ Àß Á¶ÀýµÇÁö ¾Ê´Â °íÇ÷´çÀÌ ³ªÅ¸³ª Æ÷µµ´çÀÇ °¨·®ÀÌ ÇÊ¿äÇÏ¿´À¸³ª ´Ù¸¥ ´ë»ç¼º ¹× Á¤¸Æ°üÀÇ °¨¿°°ú °ü·ÃµÈ ÇÕº´ÁõÀº ¾ø¾ú´Ù. 3) Ç÷û ÃѺô¸®·çºóÀº ÃÑ 5·Ê¿¡¼­ »ó½ÂÇÏ¿´°í Áֱ⼺ Á¤¸Æ¿µ¾ç¹ý ½ÃÇà ÈÄ Àü Áõ·Ê¿¡¼­ Á¤»óÈ­µÇ¾ú´Ù(p£¼0.05). Áֱ⼺À¸·Î Àüȯ½Ã Ç÷û ºô¸®·çºóÀº 3.6 mg/dL¿´À¸¸ç Á¤»óÈ­µÈ ¶§±îÁö °É¸° ±â°£Àº 8ÀÏÀ̾ú´Ù. 4) AST/ALTÀÇ »ó½ÂÀº 12·Ê¿¡¼­ ¸ðµÎ °üÂûµÇ¾ú°í Áֱ⼺ Á¤¸Æ¿µ¾ç¹ý ½ÃÇà ÈÄ Àü·Ê¿¡¼­ Á¤»óÈ­µÇ¾ú´Ù. Áֱ⼺À¸·Î Àüȯ½Ã Ç÷û ALT´Â 200 IU/L¿´À¸¸ç Áֱ⼺ Á¤¸Æ¿µ¾ç ½ÃÇà ÈÄ Á¤»óÈ­µÈ ¶§±îÁö °É¸° ±â°£Àº 34ÀÏÀ̾ú´Ù.

°á ·Ð: ÀúÀÚµéÀº Àå±â°£ÀÇ ÃÑÁ¤¸Æ¿µ¾ç¹ýÀ¸·Î ÀÎÇØ ¹ß»ýÇÑ °£±â´É ÀÌ»óÀ» º¸ÀÎ À¯¼Ò¾Æ¿¡¼­ Áֱ⼺ Á¤¸Æ¿µ¾ç¹ýÀ¸·ÎÀÇ Á¶±âÀüȯÀÌ È¿°úÀûÀÎ Ä¡·áÀû ´ë¾ÈÀÌ µÉ ¼ö ÀÖÀ¸¸ç ºÎÀÛ¿ëÀÌ ÀûÀº ¾ÈÀüÇÑ ¹æ¹ýÀÓÀ» °üÂûÇÏ¿´´Ù. ÇâÈÄ Áֱ⼺ Á¤¸Æ¿µ¾ç¹ýÀº ÃÑÁ¤¸Æ¿µ¾çÀ» ½ÃÇà¹Þ´Â ¼Ò¾Æµé¿¡¼­ °£±â´É ÀÌ»óÀ» ¹æÁöÇÏ°í »îÀÇ ÁúÀ» ³ôÀ̱â À§ÇÑ ÀÏÂ÷ÀûÀÎ ¹æ¹ýÀ¸·Î ´õ¿í ¿¬±¸µÇ¾î¾ß ÇÒ °ÍÀ¸·Î »ý°¢µÈ´Ù.

Purpose:Liver function test abnormalities have been reported frequently in patients receiving total parenteral nutrition (TPN). In adults, it is known that liver complications decrease with the use of cyclic parenteral nutrition (CPN), especially if the shift to cycling was not too late. However, there are few studies about the effects of cycling on liver injury in children beyond the neonatal period. The aim of this study is to evaluate the effect of the early use of CPN on total parenteral nutrition induced hepatic dysfunction.

Methods:Twelve sets of CPN in 11 children (2 months to 17 years) were included in this study. Data on underlying diseases, age, length of time on TPN, macronutrient intake, complications, and biochemical parameters were collected from clinical records. All children had received CPN in the early period of persistent transaminase elevation or cholestasis complicated by previous continuous PN. The duration of infusion off-time in CPN was 2 hours in patients less than 3 months of age and 4 hours in the older children.

Results:All 12 cases showed elevated aminotransferase and 5 of them also showed cholestasis. Serum total bilirubin concentration was normalized in all 5 cases with median periods of 8 days (p<0.05) after initiation of CPN. ALT either decreased significantly or was normalized in all cases with median periods of 30 days (p<0.05) on CPN. The CPN was well tolerated without significant complication except for one case of hyperglycemia.

Conclusion:The early use of cyclic parenteral nutrition had a beneficial effect in improving hepatic dysfunction complicated by TPN in children.
