Abstract

¸ñ Àû: H. pylori¿¡ ´ëÇؼ­´Â ÃÖ±Ù ¸¹Àº ¿¬±¸°¡ÁøÇàµÇ¾î ¼ºÀΰú ¼Ò¾Æ¿¡¼­ À§¿°°ú °­ÇÑ °ü·Ã¼ºÀÌ ÀÖÀ¸¸ç À§ Á¡¸·Àº ¹°·Ð ½ÊÀÌÁöÀå Á¡¸·¿¡¼­ À§ »óÇÇÈ­»ýÀÌ ¹ß»ýÇÏ¸é ¹Ì¶õ°ú ±Ë¾çÀÌ ¹ß»ýÇÒ ¼ö ÀÖ´Â °ÍÀ¸·Î º¸°íµÇ¾î ¿Ô´Ù. ±×·¯³ª ¼Ò¾Æ¿¡¼­ÀÇ H. pylori °¨¿°°ú °ü·ÃµÈ ½ÊÀÌÁöÀåÀÇ Á¶Á÷º´¸®ÇÐÀû ¼Ò°ß¿¡ ´ëÇÑ ¿¬±¸´Â ¾ÆÁ÷±îÁö ¹Ì¹ÌÇÑ ½ÇÁ¤À̹ǷΠº» ¿¬±¸¿¡¼­´Â H. pylori¿¡ °¨¿°µÈ ¼Ò¾Æ¿¡¼­ ½ÊÀÌÁöÀå¿°, ½ÊÀÌÁöÀå ±Ë¾ç ¹× À§ »óÇÇÈ­»ý¿¡ ´ëÇÑ °ü°è¸¦ ±Ô¸íÇغ¸°íÀÚ ÇÏ¿´´Ù.

¹æ ¹ý: 2001³â 10¿ùºÎÅÍ 2004³â 4¿ù±îÁö °¡ÃµÀÇ´ë ±æº´¿ø ¼Ò¾Æ°ú¿¡ ³»¿øÇÏ¿© À§ ÀüÁ¤ºÎ¿Í À§ üºÎ ¹× ½ÊÀÌÁöÀå Á¡¸·¿¡¼­ »ý°ËÀ» µ¿½Ã¿¡ ½ÃÇàÇÑ177¸íÀÇ È¯¾Æ¸¦ ´ë»óÀ¸·Î H. pylori °¨¿° ¿©ºÎ ¹× º´¸®ÇÐÀû ¼Ò°ßÀ» ÀüÇâÀû ¹æ¹ýÀ¸·Î Á¶»ç ºÐ¼®ÇÏ¿´´Ù. H. pylori ¾ç¼ºÀº Á¶Á÷ÀÇ Giemsa stain¿¡¼­ H.pylori°¡ ¹ß°ßµÇ¾úÀ» ¶§¿Í H. pylori IgG, urease breath test (UBT), rapid urease test (CLO) Áß 2°¡Áö À̻󿡼­ ¾ç¼ºÀÏ ¶§·Î Á¤ÀÇÇÏ¿´´Ù. ½ÊÀÌÁöÀå¿°Àº hematoxylin ¹× eosin ¿°»ö¿¡¼­ °£ÁúÀÇ ¼¼Æ÷ ħÀ± Á¤µµ¿¡

°á °ú: 1) ´ë»ó ȯ¾Æ 177¸í Áß H. pylori ¾ç¼º ¹ÝÀÀÀ» º¸ÀΠȯ¾Æ´Â 97¸í(54%)À¸·Î ³²³à ºñ´Â 1.36 :1À̾úÀ¸¸ç, Æò±Õ ³ªÀÌ´Â 9.66¼¼¿´´Ù.2) ½ÊÀÌÁöÀå¿°Àº 163¸í¿¡¼­ È®ÀεǾú´Âµ¥ 94¸í¿¡¼­ Á¦ 1µµ ¼Ò°ßÀ» º¸¿´°í ÀÌ Áß H. pylori °¨¿°Àº 47¸í À̾ú´Ù. Á¦ 2µµ´Â 62¸íÀ¸·Î ÀÌ Áß H. pylori °¨¿°Àº 36¸íÀ̾úÀ¸¸ç, Á¦ 3µµ´Â 7¸íÀ¸·Î ÀÌ Áß H. pylori °¨¿°Àº 4¸íÀ̾ú´Ù. H. pylori °¨¿°°ú ½ÊÀÌÁöÀå¿° °£¿¡´Â À¯ÀÇÇÑ »ó°ü°ü°è°¡ ¾ø¾ú´Ù.3) ½ÊÀÌÁöÀå ±Ë¾çÀº 11¸íÀ¸·Î Æò±Õ ³ªÀÌ´Â 7.18¼¼ ¿´À¸¸ç ³²³à ºñ´Â 4.5 : 1À̾ú´Ù. °á ·Ð: ±âÁ¸ÀÇ ¹®Çåµé¿¡¼­¿Í ´Þ¸® º» ¿¬±¸¿¡¼­´Â H. pylori °¨¿°°ú ½ÊÀÌÁöÀå¿°, H. pylori °¨¿°°ú ½ÊÀÌÁöÀå ±Ë¾ç ¹× H. pylori °¨¿°°ú À§ »óÇÇÈ­»ý °£¿¡´Â ¿¬°ü¼ºÀÌ ¾ø¾úÀ¸¸ç, ½ÊÀÌÁöÀå¿°°ú ½ÊÀÌÁöÀå ±Ë¾ç °£¿¡´Â ¿¬°ü¼ºÀÌ ÀÖ¾ú´Ù. ±×·¯³ª À§ »óÇÇÈ­»ý°ú ½ÊÀÌÁöÀå ±Ë¾ç ¹× ½ÊÀÌÁöÀå¿°°ú À§ »óÇÇÈ­»ý °£¿¡´Â ±âÁ¸ÀÇ º¸°íµéó·³ ¿¬°ü¼ºÀÌ ÀÖÀ½À» ¾Ë ¼ö ÀÖ¾ú´Ù.

Purpose: Helicobacter pylori (H. pylori) infection has been known to be vital in the pathogenesis of duodenal ulcer disease in children as well as in adults. But the relationship between H. pylori infection and the histopathologic findings of the duodenum has not been explained obviously in children yet. So the aim of this study is to determine whether duodenitis and/or gastric metaplasia in the duodenum increases the risk of duodenal ulcer disease in children infected by H. pylori.

Methods:From October 2001 to April 2004 gastric and duodenal biopsies were performed in 177 children who visited Department of Pediatrics, Gil Hospital, Gachon Medical School. Biopsy sections were stained with hematoxylin and eosin and also with Giemsa for identification of H. pylori. The grades of duodenitis and gastric metaplasia were classified from 0 to 3 and from 0 to 4, respectively.

Results: The incidence of H. pylori infection was 54% in total patients. Amongst 163 children with duodenitis there was a lack of correlation between H. pylori infection and the grade of duodenitis. Amongst 11 patients with duodenal ucler, only 4 children were infected by H. pylori. And amongst 5 patients with gastric metaplasia, H. pylori and duodenal ulcer were detected in 2 and 3 children, respectively. The occurrence of duodenal ulcer and gastric metaplasia were increased significantly in proportion to the grade of duodenitis (p<0.0001 and p=0.0365, respectively).

Conclsuion:As opposed to the results of previously reported articles, there were lacks of correlation between H. pylori infection and duodenitis, duodenal ulcer, and gastric metaplasia. So further study hould be done to clarify the effect of H. pylori on the duodenal histopathology in children infected by H. pylori.
