Abstract

¸ñ Àû: ÀÚ°¡¸é¿ª¼º °£¿°Àº ¿øÀÎ ºÒ¸íÀÇ ÁøÇ༺ ¿°Áõ¼º °£ÁúȯÀ¸·Î Ç÷Áß ÀÚ°¡Ç×üÀÇ ÃâÇö°ú °í°¨¸¶±Û ·ÎºÒ¸°Ç÷Áõ, ±×¸®°í Á¶Á÷ÇÐÀûÀ¸·Î ¹®¸ÆÁÖÀ§ ¿°ÁõÀ» Ư¡À¸·Î ÇÑ´Ù. ¼Ò¾Æ¿¡¼­´Â »ê¹ßÀûÀÎ Áõ·Ê º¸°í°¡ ÀÖ¾úÀ¸³ª ±¹³»¿¡¼­´Â ¾ÆÁ÷±îÁö ü°èÀûÀÎ ¿¬±¸°¡ ¾ø¾ú´Ù. ¹æ ¹ý: 1990³âºÎÅÍ 2004³â±îÁö ¼­¿ï´ëÇб³ ¾î¸°À̺´¿ø¿¡¼­ ÀÚ°¡¸é¿ª¼º °£¿°À¸·Î Áø´ÜµÇ¾ú´ø 14¸íÀÇ È¯ÀÚµéÀÇ ÀÓ»óÀû Ư¡, Ç÷¾× °Ë»ç ¼Ò°ß ¹× °£ Á¶Á÷ °Ë»ç ¼Ò°ßÀ» ºÐ¼®ÇÏ¿´´Ù. ¶ÇÇÑ À̵é ȯÀڵ鿡°Ô ½ÃÇàµÈ Ä¡·á ¾àÁ¦ ¹× Ä¡·á¿¡ÀÇ ¹ÝÀÀ, ±×¸®°í Àç¹ß

°á °ú: ´ë»ó ȯÀÚ 14¸íÀÇ Áø´Ü½Ã ¿¬·ÉÀº Æò±Õ 9¡¾3 ¼¼¿´´Ù. ¿©ÀÚ°¡ 11¸íÀ¸·Î ÀüüÀÇ 78%¸¦ Â÷ÁöÇÏ¿´°í, 6¸í¿¡¼­ ±Þ¼º ¹ßº´À» º¸¿´´Ù. ÀÓ»ó Áõ»óÀ¸·Î´Â Ȳ´ÞÀÌ 11¸í¿¡¼­ È£¼ÒÇÏ¿© °¡Àå ÈçÇÏ¿´À¸¸ç, ÀÌ ¿Ü ¿¡µµ ÇǷΰ¨, ½Ä¿åºÎÁø, ÁøÇÑ»ö ¼Òº¯, »óº¹ºÎ ºÒÄè°¨, ÀæÀº ÄÚÇÇ, ¿À½É, ¼Ò¾ç°¨ ¹× ¼³»ç°¡ µ¿¹ÝµÇ¾ú´Ù. ½Åü °ËÁø ¼Ò°ß¿¡¼­´Â °£ºñÁ¾´ë°¡ ÈçÇÏ°Ô ³ªÅ¸³µ´Ù. 6¸íÀÇ È¯ÀÚ¿¡¼­ ´Ù¸¥ ÀÚ°¡¸é¿ª¼º ÁúȯÀÌ µ¿¹ÝµÇ¾ú°í 1¸í¿¡¼­´Â ÀϵîÄ£ Áß ±×·¹À̺꽺º´ÀÇ º´·ÂÀÌ ÀÖ¾ú´Ù. Ç÷û IgG´Â Æò±Õ 2971¡¾2147 mg/dL¿´°í, AST´Â Æò±Õ 469¡¾402 U/L, ÃÑ ºô¸®·çºóÀº Æò±Õ4.9¡¾5.5 mg/dL¿´´Ù. °£ Á¶Á÷ °Ë»ç ¼Ò°ßÀ¸·Î´Â ¹®¸ÆÁÖÀ§ÀÇ ¿°Áõ(10¸í), °£¼¼Æ÷ÀÇ ·ÎÁ¦Æ® Çü¼º(3¸í), ¼Ò¿±ÀÇ ¿°Áõ(6¸í)ÀÌ ÈçÇÏ°Ô ³ªÅ¸³µ°í 2¸í¿¡¼­´Â Áø´Ü´ç½Ã ÀÌ¹Ì Á¶Á÷ÇÐÀûÀ¸·Î °£°æº¯ÀÌ µ¿¹ÝµÇ¾ú´Ù. ´ë
»ó ȯÀÚ 14¸í Áß 6¸íÀº UDCA¸¸ »ç¿ëÇÏ¸ç °üÂû Áß°£¼öÄ¡°¡ È£ÀüµÇ¾ú°í ³ª¸ÓÁö 8¸í¿¡°Ô´Â ½ºÅ×·ÎÀ̵å Ä¡·á°¡ ½ÃÇàµÇ¾ú´Ù. ÀÌÁß 7¸íÀÌ Ãʱ⠽ºÅ×·ÎÀ̵åÄ¡·á½Ã °üÇظ¦ º¸¿´À¸³ª ½ºÅ×·ÎÀ̵带 °¨·®Çϰųª ÁßÁö½Ã Àç¹ßÇÏ¿´´Ù. ½ºÅ×·ÎÀ̵å ÀçÄ¡·á°¡ ½ÃÇàµÇ¾ú°í 3¸íÀº ½ºÅ×·ÎÀÌµå °¨·® Áß °üÇØ À¯Áö ÁßÀÌ°í 2¸íÀº azathioprineÀ¸·Î °üÇØ À¯Áö ÁßÀÌ´Ù.

°á ·Ð: ÀÚ°¡ ¸é¿ª¼º °£¿°Àº ¿øÀÎ ºÒ¸íÀÇ ¼Ò¾Æ ¸¸¼º°£ ÁúȯÀÇ ¿øÀÎ Áß ÇϳªÀ̸ç, ÀûÀýÇÑ Ä¡·á·Î ¿¹ÈÄ°¡°³¼±µÉ ¼ö ÀÖ´Â °£ÁúȯÀÌ´Ù. ±×·¯¹Ç·Î ƯÈ÷ ¿©¾Æ¿¡¼­ °í°¨¸¶±Û·Îºí¸°Ç÷ÁõÀ» º¸À̰ųª ´Ù¸¥ ÀÚ°¡¸é¿ª¼º ÁúȯÀÌ µ¿¹ÝµÉ °æ¿ì ÀÚ°¡¸é¿ª¼º °£¿°À» ÀǽÉÇÏ

Purpose:Autoimmune hepatitis is a chronic inflammatory liver disease with unknown cause that is characterized by liver histology, circulating autoantibodies and increased levels of immunoglobulin G. Only sporadic reports are available on autoimmune hepatitis in children. The aim of this study was to evaluate the clinical, biochemical, and histological features, and the long-term outcome of autoimmune hepatitis in Korean children.

Methods:We reviewed the medical records of 14 children diagnosed as having autoimmune hepatitis at Seoul National University Children¡¯s Hospital from 1990 to 2004, and analyzed clinical, biochemical, and histological features, and clinical outcomes.

Results:Mean age at diagnosis was 9 years and 11 of the 14 children were female. Six children presented with acute hepatitis-like manifestations. Jaundice and fatigue were the most common symptoms. Other autoimmune diseases accompanied in 6 children. Anti-nuclear antibody was detected in 13 patients and anti-smooth muscle antibody was positive in 8. All 14 patients were type 1 autoimmune hepatitis. The main histologic findings were interface hepatitis, rosette formation, and cirrhosis. Clinical and biochemical features were improved in six patients treated with ursodeoxycholic acid. Eight patients were treated with corticosteroid alone or in combination with azathioprine and five of them are in biochemical remission.

Conclusion:Autoimmune hepatitis is an inflammatory liver disease, which has a favorable long-term outcome if it is diagnosed and treated promptly. Therefore, autoimmune hepatitis should be suspected in children with chronic hepatitis of unknown etiology, especially in female patients who show hypergammaglobulinemia or some clinical features of autoimmune disease.
