Abstract
저자들은 심와부 복통과 혈변, 양 하지의 홍반성 자반 등을 주소로 내원한 HSP 환아에서 상부 위장관 내시경적 생검을 시행한 후에 발생한 십이지장 벽내 혈종과 이로 인한 십이지장 장관 폐쇄를 관찰하였고, 다른 중증 합병증 없이 보존적인 치료를 통해 완전히 호전되었던 증례를 경험하였기에 문헌 고찰과 함께 보고한다.
Henoch-Schönlein purpura is a systemic small-vessel IgA-dominant vasculitis with involvement of skin, gastrointestinal tract, joint and kidney. Henoch-Schönlein purpura and an endoscopic biopsy are rare causes of intramural hematoma of the duodenum. We herein report a case of intramural hematoma of the duodenum and duodenal obstruction following endoscopic biopsy in an 8-year-old girl with Henoch-Schönlein purpura. Abdominal CT scan showed intramural hematoma in the second and third portions of the duodenum, which caused marked narrowing of the lumen. She was treated with supportive care and improved completely without any gastrointestinal complications. (Korean J Pediatr Gastroenterol Nutr 2006; 9: 256～261)
