소아에서 위장관 이물의 자연 배출에 대한 경험적 고찰

Abstract
목 적: 위장관 이물은 내시경적 이물 제거술이나 수술적 제거술, 또는 자연 배출을 기다리는 방법이 있으며 이물의 종류와 모양, 크기, 초기 위치에 따라 치료 방법이 달라질 수 있다. 자연 배출이 예상되는 이물에 대해서는 내시경을 시행하지 않을 수 있다. 이에 본 연구에서는 위장관 이물의 자연 배출 경우와 내시경적 제거 경우를 비교하고, 자연 배출 이물의 경우에 이물의 종류, 크기, 초기 위치에 따른 자연 배출률에 대해서 알아보았다.
방 법: 2001년 1월부터 2007년 7월까지 6년간 위장관 내 이물을 주소로 삼성서울병원 응급실을 방문한 160명을 대상으로 후향적 의무기록 조사 및 보호자 전화 면담을 통해 정보를 수집해서 각각의 이물간의 특성, 위치, 자연 배출의 시간과 빈도 등을 조사하였다.
결 과: 남녀 비는 1.46：1로 남아가 95명(59.4%), 여아가 65명(40.6%)이었다. 전체 이물의 자연 배출률은 48.0%였다. 각 이물의 종류에 따른 자연 배출률은 각각 동전 36.5%, 핀, 머리핀, 나사, 열쇠 등 길고 날카로운 이물 52.6%, 바둑돌과 구슬 83.3%, 자석 69.2%, 디스크 배터리 50.0%였다. 이중에서 동전은 크기와 위치에 따라 내시경적 제거 비율에서 많은 차이를 보여 100원 동전은 초기 식도에 걸려있을 확률이 66.7% (22/33)였고 이 중에서 내시경으로 제거한 비율은 90.9%였다. 크기가 비슷한 100원 동전, 10원 동전 그리고 바둑돌을 대상으로 조사한 나이별, 위치별 자연 배출률은 초기 이물이 위 이하에 위치했을 경우 전 연령에서 80% 이상으로 높게 조사되었다.
결 론: 소아의 위장관 이물은 이물의 종류나 크기, 위치 등에 자연 배출률이 매우 다양하다. 따라서 적응증이 될 때는 내시경을 이용한 이물 제거를 시행하고, 자연 배출률이 높은 이물은 경과를 관찰하면서 자연 배출을 기대해 볼 수 있겠다. 특히 동전이나 바둑돌과 같이 둥근 물체가 식도를 통과하여 위 또는 그 이하에 위치해 있다면 자연 배출의 가능성이 높으므로 기다려 보는 것이 좋겠다. 다만 디스크 배터리의 경우는 여러 가지 합병증을 고려했을 때 내시경적 제거술을 시행하는 것이 안전할 것으로 생각된다.
Purpose: Ingested foreign bodies are removed by endoscopy, surgery or spontaneous passage, however, the decision of therapeutic modality chosen depends on the type, size, shape and location of the ingested foreign bodies. Therefore, this study was conducted to investigate the rate and characteristics of foreign bodies that were passed spontaneously out of the intestine.
Methods: One hundred and sixty patients who visited Samsung Medical Center for treatment of gastrointestinal foreign bodies between January 2001 to July 2007 were enrolled in this study. Related data was colleted by reviewing the medical records of patients with proven foreign bodies retrospectively, as well as by conducting phone interviews with the parents of the patients. All cases were classified based on the nature and location of the ingested foreign body, as well as whether it was treated by spontaneous passage.
Results: Of the 160 cases involving the passage of foreign bodies in children were included in this study (95 boys and 65 girls), endoscopic removals, operative removals or spontaneous passages were conducted in 80, 3 and 77 patients, respectively. The spontaneous passage rates for each type of object were as follows; coins (36.5%), bead and baduk stones (83.3%), long and sharp materials (52.6%), magnets (69.2%) and disc batteries (50.0%). In cases involving round-shaped foreign bodies, such as coins, the diverse spontaneous passage rates were more diverse. When foreign bodies were stuck below esophagus, similar sized coins and baduk stones had spontaneous passage rates greater than 80% regardless of age.
Conclusion: It is better for clinicians to wait for spontaneous passage to occur in cases involving coins or round-shaped foreign bodies that are located at or below the stomach. (Korean J Pediatr Gastroenterol Nutr 2007; 10: 157∼165)
