Abstract

¸ñ Àû: Çѱ¹ÀÎ ¼Ò¾Æ¿¡¼­ ¾ç¼º ÀÏ°ú¼º °íÀλêºÐÇØÈ¿¼ÒÇ÷Áõ(Benign transient hyperphosphatasemia, BTH)ÀÇ ÀÓ»óÀû Ư¡ ¹× ÀÚ¿¬ °æ°ú¸¦ »ìÆ캸°íÀÚ ÇÏ¿´´Ù.

¹æ ¹ý: 2006³â 6¿ùºÎÅÍ 2007³â 7¿ù±îÁö ºÐ´ç¼­¿ï´ëÇб³º´¿ø ¼Ò¾Æ°ú¸¦ ³»¿øÇÑ È¯¾Æ Áß Kraut µîÀÇ Áø´Ü±âÁØ¿¡ µû¶ó BTH·Î Áø´ÜµÈ 17¸íÀÇ Àǹ«±â·Ï°ú °Ë»ç°á°ú¸¦ ÈÄÇâÀûÀ¸·Î ºÐ¼®ÇÏ¿´´Ù.

°á °ú: ÃÑ 17¸íÀÇ È¯¾Æ Áß ³²¾Æ°¡ 6¸í, ¿©¾Æ°¡ 11¸íÀ̾úÀ¸¸ç, ¿¬·ÉÀº Æò±Õ 26.9¡¾13.8°³¿ù(¹üÀ§: 9¡­49°³¿ù)À̾ú´Ù. µ¿¹ÝµÈ ÁúȯÀ¸·Î´Â °¨¿°¼º ÁúȯÀÌ 52.9%·Î °¡Àå ¸¹¾Ò°í, °èÀýÀûÀ¸·Î 9¿ù¿¡¼­ 12¿ù »çÀÌ¿¡ ´õ ³ôÀº ¹ß»ý·üÀ» º¸¿´´Ù. ù ³»¿ø ½Ã ÃøÁ¤ÇÑ Ç÷û ALP´Â Æò±Õ 2500.2¡¾1165.1 U/LÀ̾ú°í, ¸ðµç ȯ¾Æ¿¡¼­ 7ÁÖ ³»¿¡ Ç÷û ALP°¡ Á¤»ó ¶Ç´Â Á¤»ó »óÇÑÄ¡ÀÇ µÎ ¹è À̳»·Î °¨¼ÒÇÏ¿´´Ù. ALP isoenzyme °Ë»ç¸¦ ½ÃÇàÇÑ 7¸íÀÇ È¯¾Æ Áß 3¸í¿¡¼­ °£ ºÐȹ°ú °ñ ºÐȹÀÇ Áõ°¡°¡ ÀÖ¾ú°í, 4¸íÀº °ñ ºÐȹÀÇ Áõ°¡¸¸À» º¸¿´´Ù.

°á ·Ð: Çѱ¹ÀÎ ¼Ò¾Æ¿¡¼­ BTHÀÇ ÀÓ»óÀû Ư¡ ¹× °æ°ú¸¦ ÀÌÇØÇÔÀ¸·Î½á ALP »ó½Â ¼Ò°ßÀ» º¸ÀÌ´Â ¿µÀ¯¾Æ¿¡¼­ ºÒÇÊ¿äÇÑ Áø´ÜÀû °Ë»çµéÀ» Áö¾çÇϸç ÀÓ»ó °æ°úÀÇ °üÂûÀ» ÅëÇØ ALP È°¼ºµµÀÇ Á¤»óÈ­ ¿©ºÎ¸¦ È®ÀÎÇϵµ·Ï ÇÏ¿©¾ß ÇÑ´Ù.

Purpose: This study was conducted to analyze the clinical manifestation and natural course of benign transient hyperphosphatasemia (BTH) in children.

Methods: A total of 17 children diagnosed with BTH between June 2006 and July 2007, were included in this study. Clinical records and laboratory tests were analyzed retrospectively for all children.

Results: The mean age of the patients was 26.9¡¾13.8 months (range: 9 to 49 months). Nine of the 17 BTH cases (52.9%) occurred in patients that had previously suffered from infectious diseases. While the patients were suffering from BTH, the mean serum level of alkaline phosphatase (ALP) was 2,500.2¡¾ 1,165.1 U/L, however, these elevated serum ALP levels normalized within 7 weeks of recovery in all cases. In addition, a seasonal peak in the onset of BTH was observed from September to December. Further, the measurement of ALP isoenzymes in 7 of the patients with BTH, revealed a marked elevation of bone fraction.

Conclusion: It is important for pediatricians to understand the clinical manifestation and benign course of BTH to avoid unnecessary diagnostic evaluation in children showing a transient increase in ALP activities. (Korean J Pediatr Gastroenterol Nutr 2007; 10: 173¡­178)
