직장암의 근치적 수술 후 방사선치료와 화학요법의 순서에 대한 고찰

Abstract
목적 : 근치적 수술을 시행받은 직장암 환자에서 수술 후 보조치료로서 화학요법과 방사선치료를
시행시, 방사선치료와 화학요법의 시행 시점에 따른 부작용과 치료 실패 양상, 생존율을 비교하고
자 전향적 무작위 3상 임상연구를 시행하여 가장 적절한 화학요법과 방사선치료 순서를 결정하고
자 하였다.
대상 및 방법 : 1996년 1월부터 1999년 3월까지 근치적 절제술을 시행받은 AJCC 병기 2기 및 3
기의 직장암 환자 313명을 대상으로 조기 방사선 치료군(arm I)과 지연 방사선 치료군 (arm Ⅱ)
으로 나누어 방사선치료와 화학요법 병용치료를 시행하였다. 화학요법은 5-FU 375
mg/m2/day와 leucovorin 20 mg/m2/day를 방사선치료와 동시치료 기
간에는 3일간씩, 화학요법 단독 기간 동안에는 5일간씩 총 8회를 정맥주사 하였고 방사선치료는
전골반 영역에 45 Gy/25 fractions/5 weeks를 시행하였다. 이 중 1998년 6월까지 등록된 228명을
대상으로 중간 분석을 시행하였고 228명중 두 명의 환자는 이차적인 원발종양의 발생으로 분석에
서 제외되었다. Arm l은 1회차 화학요법과 동시에 방사선치료를 시행하였고, Arm Ⅱ는 화학요법
을 2회 마친 후 3회차와 동시에 시행하였다. 중앙추적관찰기간은 23개월이었다.
결과 : 국소재발은 Arm l에서 11명(9.7%), Arm Ⅱ에서 9명(8%)이었고 두 구간에 통계적으로 의
미있는 차이는 없었으나 원격전이는 Arm l에서 22명(19.5), Arm Ⅱ에서 35명(31%)으로 의미있게
Arm l에서 낮게 나타났다(p=0.046). 그러나 3년 무병생존율(70.2% vs 59.2%, p=0.2)과 3년 생존
율(89.4% 88.0%, p=0.47)은 유의한 차이가 없었다. 방사선치료 중 RTOG grade 1 이상의 백혈구
감소는 78.3에서 관찰되었고 방사선치료 후 화학요법 기간 동안에는 79.9%에서 관찰되었으나,
RTOG grade 3 이상의 백혈구 감소증은 각각 2.1%, 6%로 매우 낮게 나타났다. 저위전방절제술을
시행받은 환자 중 하루 10회 이상의 설사를 한 환자는 Arm l에서 71.2%, Arm Ⅱ에서 42.6%로
의미있게 Arm l에서 높았으나(p=0.02) 이러한 부작용은 보조적인 치료로 회복되었다.
결론 : 근치적 절제술 후 그 순서에 관계없이 방사선치료와 화학요법을 시행함으로써 국소재발율
을 낮출 수 있었다. 원격전이는 수술 후 조기 방사선치료군에서 더 낮은 것으로 나타났는데 뚜렷
한 원인은 밝힐 수 없었고 향후 추적 관찰이 더 필요할 것으로 생각된다. 본 치료는 대부분의 환
자가 큰 부작용 없이 마쳐 비교적 안전한 치료임을 확인할 수 있었지만, 치료 순응도는 좀 더 높
아져야 할 것으로 생각된다.
Purpose : To evaluate the side effects, pattern of failure, and survival rate according to the
sequence of postoperative adjuvant radiotherapy and chemotherapy, patients with stages Ⅱ
and Ⅲ rectal cancer who had undergone curative resection were randomized to‘early
radiotherapy group (arm Ⅰ)’or‘late radiotherapy group (arm Ⅱ)’, then we intend to
determine the most effective sequence of the radiotherapy and chemotherapy.
Materials and Methods : From January 1996 to March 1999, 313 patients with curatively
resected stages Ⅱ and Ⅲ rectal cancer have been randomized to early or late radiation
therapy group and received combined chemotherapy (5-FU 375 ㎎/m2/day,
leucovorin 20 ㎎/m2, Ⅳ bolus daily D1-5, 8 cycles) and radiation therapy (whole
pelvis with 45 Gy/25 fractions/5 weeks). Arm Ⅰ received radiation therapy from day 1 with
first cycle of chemotherapy and arm Ⅱ received radiation therapy from day 57 with third
cycle of chemotherapy after completion of first two cycles. Preliminary analysis was
performed with 228 patients registered up to Jun 1998. Two out of the 228 patients were
excluded because of double primary cancer. Median follow-up period was 23 months.
Results : Local recurrence occurred in 11 patients (9.7％) for arm Ⅰ and 9 patients (8％) for
arm Ⅱ. There was no significant difference between both groups (p=0.64). However, distant
metastasis was found in 22 patients (19.5％) for arm Ⅰ and 35 patients (31.0％) form arm Ⅱ
and which showed statistically significant difference between the two groups (p=0.046). And
neither 3-year disease-free survival (70.2％ vs 59.2％, p=0.2) nor overall survival (89.4％ vs
88.0％, p=0.47) showed significant differences. The incidence of leukopenia during radiation
therapy and chemotherapy was 78.3％ and 79.9％ respectively but leukopenia more than
RTOG grade 3 was only 2.1％ and 6.0％ respectively. The incidence of diarrhea more than 10
times per day was significantly higher in the patients for arm Ⅰthan for arm Ⅱ (71.2％ vs
41.6％, p=0.02) but this complication was controlled with supportive cares.
Conclusion : Regardless of the sequence of postoperative adjuvant radiation therapy and
chemotherapy after curative resection for rectal cancer, local recurrence rate was low with
combined chemoradiotherapy. But distant metastasis rate was lower in early radiation therapy
group than in late radiation therapy group and the reason is unclear. Most patients completed
these treatments without severe complication, so these were thought to be safe treatments but
the treatment compliance should be impoved.
