Abstract

¸ñ Àû: ¼ö¼ú ÈÄ ¹æ»ç¼± Ä¡·á¸¦ ¹ÞÀº Èĺ¹¸· ¿¬Á¶Á÷ À°Á¾ ȯÀÚµéÀÇ Ä¡·á ¼ºÀû°ú ¿¹ÈÄ ÀÎÀÚ¸¦ ºÐ¼®ÇØ º¸°íÀÚ ÇÏ¿´´Ù.

´ë»ó ¹× ¹æ¹ý: 1985³âºÎÅÍ 2003³â±îÁö ¼ö¼ú ÈÄ ¹æ»ç¼± Ä¡·á¸¦ ¹ÞÀº Èĺ¹¸· ¿¬Á¶Á÷ À°Á¾ ȯÀÚ 23¸íÀÇ ÀڷḦ ºÐ¼®ÇÏ¿´´Ù. ÃßÀû °üÂû ±â°£Àº 8°³¿ù¿¡¼­ 240°³¿ù±îÁö·Î Áß¾Ó°ªÀº 77°³¿ùÀ̾ú´Ù. Àüü ȯÀÚ Áß 21¸íÀÌ ¿ø¹ß¼º Á¾¾çÀ̾ú°í, µÎ ¸íÀº Àç¹ß¼º À̾ú´Ù. Áö¹æÀ°Á¾°ú ÆòÈ°±ÙÀ°Á¾ÀÌ Àüü Á¾¾çÀÇ 78%¸¦ Â÷ÁöÇÏ¿´°í 17¸íÀÇ È¯ÀÚ°¡ Á¶Á÷ÇÐÀûÀ¸·Î °íµî±Þ(2¶Ç´Â 3µî±Þ)À̾ú´Ù. Á¾¾çÀÇ Å©±â´Â 3¿¡¼­ 50 cm±îÁö ºÐÆ÷ÇÏ¿´À¸¸ç Áß¾Ó°ªÀº 13 cmÀ̾ú´Ù. 65%ÀÇ È¯ÀÚ¿¡¼­ Á¾¾çÀÇ ¿ÏÀüÀýÁ¦°¡ ÀÌ·ç¾îÁ³´Ù. ¹æ»ç¼±Ä¡·á´Â ÀÏÀÏ 1.8¿¡¼­ 2.0 Gy¸¦ ÁÖ 5ȸ Á¶»çÇÏ¿© ÃÑ 45¿¡¼­ 59.4 Gy¸¦ Á¶»çÇÏ¿´À¸¸ç Áß¾Ó°ªÀº 50.4 Gy¿´´Ù.

°á °ú: ȯÀÚµéÀÇ 5³â Àüü»ýÁ¸À², ±¹¼Ò¹«Àç¹ß»ýÁ¸À², ¹«ÀüÀÌ»ýÁ¸À²Àº °¢°¢ 68%, 58%, 71%¿´´Ù. 11¸íÀÇ È¯ÀÚ¿¡¼­ ±¹

Purpose: To evaluate the clinical outcomes and prognostic factors in retroperitoneal soft tissue sarcomas treated by postoperative radiotherapy.

Materials and Methods: The records of 23 patients with retroperitoneal soft tissue sarcomas, who underwent
postoperative radiotherapy between 1985 and 2003, were analyzed. The median follow-up period was 77months (range, 8¡­240 months). A total of 21 patients presented with primary disease, and two patients presented with recurrent disease. Liposarcomas and leiomyosarcomas represented 78% of the diagnosed tumor cases. Moreover, 17 cases were of high grade (grade 2 or 3). The median tumor size was 13 cm (range, 3¡­50 cm). Complete excision was achieved in 65% of patients. The median radiation dose was 50.4 Gy (range, 45.0 to 59.4 Gy), with conventional fractionation.

Results: The 5-year overall, local recurrence-free, and distant metastasis-free survival rates were 68%, 58%, and 71%, respectively. Eleven patients experienced local recurrence, while 9 patients experienced distant metastasis. The ost common site for distant metastasis was the liver. A univariate analysis revealed that adjacent organ invasion and age (£¾60 years) as the significant risk factors contributing to the prediction of poor overall survival. Moreover, multivariate analyses indicated that adjacent organ invasion remained significantly associated with a higher risk of death. In addition, patient age (£¾60 years) was the other identified risk factor for local recurrence by univariate and multivariate analyses. Except for one case of grade 3 diarrhea, no patient suffered grade 3 or higher complications.

Conclusion: Our results were comparable to previous reports in that adjacent organ invasion and patient age
(£¾60 years) were significant predictors of poor survival and tumor recurrence, respectively.
