Abstract
목적: 노인인구의 증가에 따라 노인 당뇨병 유병률이 증가 할 것으로 예상되나 한국에서는 아직 통계자료가 부족하다. 이에 저자는 Southwest Seoul (SWS) 연구의 일환으로 서울 서남부지역 60세 이상의 노인 인구에서 1999년과 2002년의 3년간 당뇨병 유병률의 변화와 발생률을 조사하였다.
방법: 1999년 연구는 서울 서남부지역에 거주하고있는 60세 이상의 노인 1,652명이 최종적으로 분석되었고 공복혈당, 75 g 경구혈당 부하검사, 생화학적 검사, 신체계측 그리고 병력청취를 시행하였다. 2002년에는 같은 지역에서 같은 방법으로 조사된 60세 이상의 노인 1,214명이 분석되었다. 1999년도부터 2002년까지 3년간 350명이 추적관찰되었고, 그 중 기초조사에서 당뇨병이 없었던 294명이 코호트 연구에 참여하였다.
결과: 1999년 연구에서 1,652명 중 142명(8.6%)이 새로 당뇨병을 진단받았고, 당뇨병 과거력이 있는 군은 196명(11.9%)으로 전체 당뇨병 유병률은 20.5%였다. 2002년 연구에서는 1,214명 중 84명(6.9%)이 새로 당뇨를 진단받았고, 당뇨 과거력이 있는 군은 185명(15.2%)으로 전체 당뇨병 유병률은 22.1%였다. 1999년 연구에서 공복혈당장애 또는 내당능장애를 포함한 당대사 이상의 유병률은 22.8%이었고, 2002년에는 27.8%이었다. 3년간 추적관찰된 350명을 대상으로 분석한 결과에서도 당뇨병 유병률이 연령증가에 따라 1999년 16.0%에서 2002년 23.4%로 증가하였다(p=0.014). 코호트 연구에서 당뇨병 연간 조발생률은 2.9%, 성과 연령을 보정한 결과는 2.5%였다.
결론: 서울 서남부지역 60세 이상의 노인인구의 당뇨병 유병률은 1999년 20.5%에서 2002년 22.1%로 증가되었고, 공복혈당장애(IFG) 또는 내당능장애(IGT)를 포함한 당대사 이상의 유병률도 22.8%에서 27.8%로 증가하였다. 이상의 결과로 당뇨병 등의 당대사 이상이 한국도심 노인인구에서 증가함이 관찰되었다.
Background: As elderly people increasing, prevalence of diabetes will increase but there was paucity of data on the epidemiology of diabetes in Korean elderly population. In this study (Southwest Seoul, SWS study) we investigated the change of prevalence of diabetes mellitus for two points in time (the years 1999 and 2002) and 3-year incidence of diabetes in elderly Korean population of southwest area of Seoul.
Methods: A sampling of 1,652 subjects in 1999 and 1,214 subjects in 2002 who are over 60 years old in southwest area of Seoul were investigated. All subjects underwent a 75 g oral glucose tolerance test (OGTT), biochemical study and anthropometric measurements. Among the 1,652 subjects in 1999, 350 subjects were followed up for 3 years and 294 subjects without diabetes at baseline examination participated in retrospective cohort study.
Results: Prevalence of diabetes in 1999 was 20.5% [previous diagnosed people (11.9%), newly diagnosed people (8.6%)] and in 2002 was 22.1% [previous diagnosed people (15.2%), newly diagnosed people (6.9%)]. Prevalence of impaired fasting glucose (IFG) or impaired glucose intolerance (IGT) was 22.8% in 1999 and 27.8% in 2002. In 350 subjects were followed up for 3 years, prevalence of diabetes was 16.0% in 1999 and increased to 23.4% in 2002 (p=0.014). In retrospective cohort study, the annual incidence rate of diabetes was 2.9% (age and sex adjusted rate 2.5%).
Conclusion: Prevalence of diabetes in elderly Korean population of southwest area of Seoul was 20.5% in 1999 and increased to 22.1% in 2002. Prevalence of IFG or IGT was 22.8% in 1999 and increased to 27.8% in 2002. This report supports that impaired glucose regulations and diabetes mellitus of elderly Korean population will increase in the future as aged Korean population increases.
