Abstract
목적 : 이번 연구는 베체트 장염과 베체트병의 진단기준을 만족하지 않는 단순성 궤양의 임상양상을 비교분석하고자 한다.
방법 : 임상적으로 진단 기준과 상관없이 베체트 장염을 의심한 52명의 환자들의 의무 기록을 후향적으로 분석하였다. 대상 환자 중 27명(1군)은 국제 베체트병 연구 그룹(International Study Group for Behcet’s Disease) 과 일본 베체트 구회(Behcet’s disease Research Committee of Japan)에서 제시한 진단 기준을 모두 만족하였으며, 13명(2군)은 후자만 만족하였고, 두 기준을 모두 만족하지 않는 12명(3군)은 단순성 궤양으로 간주 하였다. 내과적 치료에 대한 반응은 소화기 증상과 추적
대장 내시경검사 소견으로 판단하였으며, 수술 치료를 한 경우에 수술률과 재수술률을 비교 분석하였다.
결과 : 대상 환자는 남자 27명, 여자 25명으로 남녀비는 1.08 : 1이였다. 장관 궤양 진단 당시의 평균 연령은 38.6±12.2세였으며 평균 추적관찰기간은 35.2±39.5개월 이었다. 대장 내시경검사 소견에서 회장 말단에 위치한 변연에 발적이 있는 원형 또는 난원형의 얕은 단일 궤양 이 가장 흔한 형태였으며, 각 군 간 대장 내시경검사 소견은 차이가 없었다. 평균 약물 치료 기간은 19.6±24.4 개월이었으며, 소화기 증상을 기준으로 19명(44%), 추적 대장 내시경검사 결과 10명(39%)가 완전관해에 도달하였다. 전체 대상자 중 11명(21.2%)이 수술 치료를 받았는데, 누적 수술률은 증상 발현 시점으로부터 2년에 9.9%, 5년에 17.1%, 10년에 40.5%였으며, 재수술률은 2 년에 29.4%, 5년에 43.7%, 10년에 71.8%였다. 약물 치료 반응, 누적 수술률 및 재수술률에 있어서 세 군 간 통계적 유의성은 없었으나 베체트 장염에서 HLA-B51 양성률이 유의하게 높았다(p=0.005).
결론 : 베체트 장염과 단순성 궤양은 임상양상에 있어서 차이점을 보이지 않았다. 따라서 베체트병의 진단 기준을 완전히 만족하지 않는 단순성 궤양 환자도 때로는 베체트 장염과 마찬가지로 치명적인 경과를 밟을 수 있다는 것을 염두에 두어야 하겠다.
Background : This study was performed to compare the clinical outcomes of intestinal Behcet’s disease with a simple ulcer.
Methods : We analyzed the medical records of 52 patients that were suspected as having intestinal Behcet’s disease. Of these patients, 27 patients (Group 1) met both the criteria of the International Study Group for Behcet’s Disease and the Behcet’s Disease Research Committee of Japan. Thirteen patients (Group 2) met only the latter criteria and the other patients (Group 3) did not meet any criteria. The efficacy of medical treatment was assessed by the presence of gastrointestinal symptoms and follow-up colonoscopic findings.
Results : The mean age for patients with a diagnosis of an intestinal lesion was 38.6±12.2 years. The sex ratio was 1.08:1 (M:F) and the mean follow-up duration was 35.2±39.5 months. A single, smaller than 5 mm, round and shallow ulcer with an erythematous margin that was located at the leocecal area showed most typical colonoscopic features for intestinal Behcet’s disease. No significant differences were found in the clinical manifestations and colonoscopic findings among the three groups of patients. Nineteen (44%) patients achieved complere remission from a sumptomatic point of view and 10 (39%) patients were proved to be complete remission according to follow up colonoscopy after medical treatment. Eleven patients (21.2%) underwent surgery. The overall cumulative rates of a first surgery and re-surgery were 40.5% and 71.9% at 10 years. No statistical relationship was found in the response of medical treatment and the cumulative rate of surgery among the groups.
Conclusions : The clinical course and outcomes of an intestinal simple ulcer are not different from that for intestinal Behcet’s disease.
