영상의학과 의사들을 위한 유방암 검진의 이해

Abstract
유방촬영술 검진은 유방암 사망률을 감소시킨다고 입증되어 일차적인 검진 방법으로 사용된다. 하지만, 최근 위양성결과와 과진단 등의 위해성에 대한 우려의 목소리가 높아졌고 이를 반영하여 유방촬영술 검진 가이드라인이 개정되었다. 유방초음파 검진은 치밀 유방으로 인해 유방촬영술에서 보이지 않는 유방암을 발견할 수 있는 장점이 있지만, 위양성결과 및 조직 검사를 증가시켜 의료 비용을 상승시키는 문제점이 있다. 현재까지 유방초음파 검진에 대한 권고사항은 정립되어 있지 않다. 자기공명영상 검진은 유방암 발생 고위험군에서 권고되며 우수한 민감도를 갖는 장점이 있지만, 접근성이 떨어지고 고가의 검사 비용의 문제점이 있다. 이 논문에서는 유방암 검진에 일반적으로 이용되는 영상 검사 방법의 장단점을 알아보고 검진 가이드라인을 검토하고자 한다. 또한 개개인의 특성을 고려한 맞춤 유방 검진의 방법으로 알려져 있는 유방암 발생 위험률에 따른 검진을 제시하고자 한다.
Mammography screening has been shown to reduce breast cancer mortality and remains the primary screening method. However, the harms of false-positive results and over-diagnosis have recently been emphasized, and screening guidelines have been revised. Breast ultrasonography screening has the advantage of detecting mammographically occult breast cancers in dense breasts; however, false-positive results and rising medical costs due to increased biopsy recommendations create a problem. Till date, recommendations for breast ultrasonography screenings have not been established. Magnetic resonance imaging is recommended for women at high risk and has excellent sensitivity, but it is expensive and inferior in accessibility. This article reviews the benefits, harms, and guidelines of methods commonly used in breast cancer screening. This review also provides breast cancer screening methods according to individual breast cancer risk.
