Abstract

¸ñÀû: 5 mm ÀÌÇÏ ÀÛÀº À¯¹æ¾ÏÀÇ ÃÊÀ½ÆÄ ¼Ò°ß°ú ÀÓ»óº´¸®ÇÐÀû ¼Ò°ß, ¸é¿ª Á¶Á÷ È­ÇÐÀû ¼Ò°ßÀ» ºñ±³ ºÐ¼®ÇÏ¿© 5 mm ÃÊ°úÀÇ Å« À¯¹æ¾Ï°úÀÇ Æ¯Â¡ÀûÀÎ Â÷À̸¦ ¾Ë±â À§ÇÔÀÌ´Ù.

´ë»ó°ú ¹æ¹ý: 475¸íÀÇ È¯ÀÚ¿¡¼­ 528°³ÀÇ Ä§½ÀÀ¯¹æ¾ÏÀ» ¿¬±¸ÇÏ¿´°í ÃÊÀ½ÆÄ ¼Ò°ß°ú ÀÓ»óº´¸®ÇÐÀû ¼Ò°ß, ¸é¿ª Á¶Á÷ È­ÇÐÀû ¼Ò°ßÀ» ¼ö¼ú·Î ¾òÀº Á¶½ÄÀÇ º´¸®ÇÐÀû Å©±â¿¡ µû¶ó ºÐ¼®ÇÏ¿´´Ù. ÃÊÀ½ÆÄ ¼Ò°ßÀº Breast Imaging-Reporting and Data System¿¡¼­ ±â¼úµÈ US ¼Ò°ß°ú ÃÖÁ¾ ¹üÁÖ¸¦ ÀÌ¿ëÇÏ¿´´Ù.

°á°ú: 528°³ÀÇ Ä§½ÀÀ¯¹æ¾Ï Áß 62°³ÀÇ ¹Ì¼¼Ä§½ÀÀ¯¹æ¾ÏÀÌ ÀÖ¾ú°í ´Ùº¯·® ºÐ¼®(multivariate analysis)À» ÀÌ¿ëÇØ ºÐ¼®ÇÏ¿´´Ù. ÀÛÀº À¯¹æ¾Ï¿¡¼­´Â Å« À¯¹æ¾Ïº¸´Ù ³ôÀº ºñÀ²·Î pararell orientation, not circumscribe margin, isoechoic pattern, no posterior feature, minimal vascularity ¼Ò°ßÀ» º¸¿´´Ù. ¶ÇÇÑ ÃÖÁ¾ ¹üÁÖ´Â 4¿¡ ¼ÓÇÏ´Â °æ¿ì°¡ ¸¹¾Ò´Ù. ÀÛÀº À¯¹æ¾ÏÀº Ãʱ⠴ܰèÀÎ °æ¿ì°¡ ¸¹°í Áõ»óÀ» º¸ÀÌÁö ¾Ê¾ÒÀ¸¸ç ductal carcinoma in situ component¸¦ Æ÷ÇÔÇÑ °æ¿ì°¡ ¸¹¾Ò´Ù. º´¸®ÇÐÀû ¼Ò°ßÀº Å« À¯¹æ¾Ï°ú Â÷ÀÌ°¡ ¾ø¾ú´Ù.

°á·Ð: ÀÛÀº À¯¹æ¾ÏÀÇ °æ¿ì Å« À¯¹æ¾Ï°ú ÃÊÀ½ÆÄ ¼Ò°ßÀÌ È®¿¬È÷ ´Þ¶ú´Ù.

Purpose: To identify differences in ultrasonography (US) feature, clinical and pathological characteristics including immunohistochemical characteristics between small breast cancer (pathologic size ¡Â 5 mm) and large breast cancer (> 5 mm).

Materials and Methods: A total of 528 invasive breast cancer lesions in 475 patients were included. US features with clinical and pathological characteristics were evaluated according to pathologic size. US Breast Imaging-Reporting and Data System findings and final assessments were recorded for each lesion. Standard references were based on surgical pathologies.

Results: Of 528 invasive breast cancer lesions, 62 were small breast cancers. Small breast cancers showed a higher rate of oval, round shape, parallel orientation; circumscribed margin; and iso/solid and cystic echo pattern, with no posterior feature. The final assessment of category 4 was also a dominant factor in small breast cancer. Early stage, asymptomatic state, and extensive ductal carcinoma in situ component were associated with small breast cancers.

Conclusion: Our results show that small breast cancers have less suspicious US features than large breast cancers.
