Abstract

¸ñÀû: Ç÷¾×Åõ¼®¿ë ÀÚ°¡Ç÷°ü µ¿Á¤¸Æ·çÀÇ Ç÷ÀüÀ» µ¿¹ÝÇÑ Æó»ö¿¡¼­ °æÇÇÀû ¼öµ¿ ÈíÀÎ Ç÷ÀüÁ¦°Å¼úÀÇ ¾ÈÀü¼º°ú À¯¿ë¼º¿¡ ´ëÇØ Æò°¡ÇÏ°íÀÚ ÇÑ´Ù.

´ë»ó°ú ¹æ¹ý: 2012³â 3¿ùºÎÅÍ 2017³â 12¿ù±îÁö ÀÚ°¡Ç÷°ü µ¿Á¤¸Æ·çÀÇ Ç÷ÀüÀ» µ¿¹ÝÇÑ Æó»öÀ¸·Î ³»¿øÇÏ¿© °æÇÇÀû Ç÷°ü ³» ½Ã¼úÀ» ½ÃÇà ¹ÞÀº 20¸íÀÇ È¯ÀÚ¸¦ ÈÄÇâÀûÀ¸·Î ºÐ¼®ÇÏ¿´´Ù. ±â¼úÀû ¹× ÀÓ»óÀû ½Ã¼ú ¼º°ø·ü, 1Â÷ ¹× 2Â÷ °³Åë·ü, ±×¸®°í ½Ã¼ú¿¡ µû¸¥ ÇÕº´ÁõÀ» ºÐ¼®ÇÏ¿´´Ù.

°á°ú: ÃÑ 20¸íÀÇ È¯ÀÚ¿¡¼­ 20ȸÀÇ °æÇÇÀû Ç÷°ü ³» ½Ã¼úÀ» ½ÃÇàÇÏ¿´´Ù. ¸ðµç ȯÀÚ¿¡¼­ Ç÷ÀüÀ» Á¦°ÅÇϱâ À§ÇØ °æÇÇÀû ¼öµ¿ ÈíÀÎ Ç÷ÀüÁ¦°Å¼úÀ» ½ÃÇàÇÏ¿´°í, ÇùÂøÀÌ µ¿¹ÝµÈ °æ¿ì dz¼±Ä«Å×Å͸¦ ÀÌ¿ëÇÑ Ç÷°ü¼ºÇü¼úÀ» ÇÔ²² ½ÃÇàÇÏ¿´´Ù. 17¸íÀÇ È¯ÀÚ´Â Ç÷ÀüÀÌ Á¦°ÅµÇ¾î Ç÷·ù°¡ Á¤»óÀûÀ¸·Î ȸº¹µÇ¾ú°í(±â¼úÀû ¼º°ø·ü 85%), 17¸í ȯÀÚ ¸ðµÎ ½Ã¼ú Á÷ÈÄ Ç÷¾×Åõ¼®ÀÌ Á¤»óÀûÀ¸·Î ½ÃÇàµÇ¾ú´Ù(ÀÓ»óÀû ¼º°ø·ü 85%). µ¿Á¤¸Æ·ç À§Ä¡´Â ÁÂÃø ¿ä°ñµÎÁ¤¸Æ·ç°¡ 13¸í, ÁÂÃø »ó¿ÏµÎÁ¤¸Æ·ç°¡ 4¸í, ¿ìÃø ¿ä°ñµÎÁ¤¸Æ·ç°¡ 3¸íÀ̾ú´Ù. µ¿¹ÝµÈ ÇùÂø ºÎÀ§´Â ¹®ÇպΠÁÖÀ§ Á¤¸ÆÀÌ 16¿¹, ¿øÀ§ºÎ ¹èÃâÁ¤¸ÆÀÌ 12¿¹, Áß½ÉÁ¤¸ÆÀÌ 4¿¹¿´´Ù. 1Â÷ ¹× 2Â÷ °³Åë·üÀº 1, 3, 6, 12°³¿ù¿¡¼­ °¢°¢ 100%, 70.6%, 70.6%, 56.5% ¹× 100%, 94.1%, 94.1%, 86.9%¿´´Ù. ½Ã¼ú°ú °ü·ÃµÈ ÇÕº´ÁõÀº ¾ø¾ú´Ù.

°á·Ð: Ç÷¾×Åõ¼®¿ë ÀÚ°¡Ç÷°ü µ¿Á¤¸Æ·ç¿¡¼­ Ç÷ÀüÀ» µ¿¹ÝÇÑ Æó»öÀÇ Ä¡·á·Î¼­ °æÇÇÀû ¼öµ¿ ÈíÀÎ Ç÷ÀüÁ¦°Å¼úÀº ¾ÈÀüÇÏ°í À¯¿ëÇÑ ½Ã¼úÀÌ´Ù.

Purpose: To evaluate the safety and efficacy of the percutaneous manual aspiration thrombectomy technique to treat thrombotic occlusion of native arteriovenous fistulas.

Materials and Methods: A retrospective review of 20 patients who underwent percutaneous manual aspiration thrombectomy for native thrombotic arteriovenous fistula occlusion from March 2012 to December 2017 was performed. We evaluated technical and clinical success rates and complications. The primary and secondary patency rates were calculated using the Kaplan-Meier analysis.

Results: Percutaneous manual aspiration thrombectomy was performed in 20 patients (n = 20) with concomitant balloon angioplasty. The overall technical and clinical success rates were both 85% (n = 17). The native arteriovenous fistulas, based on their site, were the left radiocephalic (n = 13), left brachiocephalic (n = 4), and right radiocephalic (n = 3) fistulas. An underlying stenosis was detected in the juxta-anastomotic venous site (n = 16), outflow draining vein (n = 12), and central vein (n = 4). The primary and secondary patency rates at 1, 3, 6, and 12 months were 100%, 70.6%, 70.6%, and 56.5% and 100%, 94.1%, 94.1%, and 86.9%, respectively. There were no complications associated with procedure.

Conclusion: Percutaneous manual aspiration thrombectomy is a safe and effective method to treat thrombotic native arteriovenous fistula occlusion.
