Abstract

¸ñÀû: °æÃß Àü»êÈ­´ÜÃþÃÔ¿µ(cervical spine CT; ÀÌÇÏ C-spine CT)°ú ÀϹÝÃÔ¿µÀ» ÀÌ¿ëÇÏ¿© ȯÃßȾÀδë°ñÈ­Áõ(ossification of the transverse ligament of the atlas; ÀÌÇÏ OTLA)°ú ¿¬°üµÈ ¿µ»ó ¼Ò°ß¿¡ ´ëÇÏ¿© ¾Ë¾Æº¸¾Ò´Ù.

´ë»ó°ú ¹æ¹ý: 11³â°£ 3975¸íÀÇ È¯ÀÚ¿¡°Ô¼­ ÃÔ¿µÇÑ 5201°³ÀÇ C-spine CT¸¦ ºÐ¼®ÇÏ¿© OTLAÀÇ À¯¹«¸¦ È®ÀÎÇÏ¿´°í, À̸¦ ´ëÁ¶±º°ú ºñ±³ÇÏ¿© ±× ºóµµ¿Í ¿¬°üµÈ ¿µ»ó ¼Ò°ßÀÇ Åë°èÇÐÀû À¯ÀǼºÀ» È®ÀÎÇÏ¿´´Ù.

°á°ú: 3975¸í Áß 45¸í¿¡¼­ OTLA°¡ È®ÀεǾú´Ù(1.1%). ±× ºóµµ´Â ³ªÀÌ¿¡ µû¶ó Áõ°¡ÇÏ´Â ¼Ò°ßÀ» º¸¿´À¸¸ç(p < 0.005), 80¼¼ ÀÌ»óÀΠȯÀÚÀÇ 12%¿¡¼­ °üÂûµÇ¾ú´Ù. ô¼ö °ø°£(space available for spinal cord; ÀÌÇÏ SAC)Àº OTLA ȯÀÚ±º¿¡¼­ À¯ÀÇÇÏ°Ô ÀÛÀº °ÍÀ¸·Î È®ÀεǾúÀ¸¸ç(p < 0.005), Àü¹æȯÃßÈĵθ·(anterior atlantooccipital membrane)-Barkow ÀÎ´ë º¹ÇÕüÀÇ ±¤¹°ÁúħÂø(mineralization), Ȳ»öÀδë°ñÈ­Áõ(ossification of ligamentum flavum), °æÃßÈĸ¸Áõ ¶ÇÇÑ À¯ÀǹÌÇÑ ¾çÀÇ »ó°ü°ü°è¸¦ º¸¿´´Ù(p < 0.005).

°á·Ð: OTLA´Â ȯÀÚÀÇ ³ªÀÌ, SAC Çù¼Ò, °æÃßÈĸ¸Áõ, ´Ù¸¥ °æÃßÀδëÀÇ °ñÈ­Áõ°ú ¿¬°üµÇ¾î ÅðÇ༺ ôÃߺ´Áõ, Àü½ÅÀûÀÎ °ñ °úÀ× »óÅÂ, ȤÀº ±â°èÀû ½ºÆ®·¹½º³ª ºÒ¾ÈÁ¤¼º¿¡ °ü·ÃÀÌ ÀÖÀ» °ÍÀ¸·Î »ý°¢µÈ´Ù.

Purpose: To determine the frequency of ossification of the transverse ligament of the atlas (OTLA) and to investigate the associated findings on cervical spine CT and plain radiography.

Materials and Methods: We reviewed 5201 CT scans of the cervical spine of 3975 consecutive patients over an 11-year period for the presence of OTLA and compared them with those of age- and sex-matched controls. The frequency and associated findings of OTLA were investigated and statistically correlated.

Results: The overall frequency of OTLA was 1.1% (45 of 3975 patients) and increased with age (p < 0.005). The frequency of OTLA in patients over 80 years was 12%. The space available for the spinal cord (SAC) was smaller in patients with OTLA (p < 0.005). Mineralization of the complex of the anterior atlantooccipital membrane and Barkow ligament, ossification of the ligamentum flavum, and kyphosis of the cervical spine positively correlated to the presence of OTLA (p < 0.005).

Conclusion: OTLA was associated with age, SAC narrowing, cervical kyphosis, and ossification of other cervical ligaments and may be associated with degenerative spondylosis, systemic hyperostotic status, or mechanical stress or instability.
