Abstract
목적 : 본 연구는 동공크기 변화에 따른 굴절이상과 고위수차를 노안 전후의 연령군으로 분류하고 비교 분석하여 노안 처방 등의 임상에 도움이 될 수 있는 정보를 제공하고자 한다.
방법 : 안질환 및 굴절교정 수술을 받은 경험이 없는 20세~80세의 성인 남녀를 20~40세 그룹과 45~80세 두 그룹으로 나누어 굴절이상과 고위수차를 측정하였다. 동공크기 변화에 따른 굴절이상과 고위수차의 비교는 Paired-T test를 이용하였고, 연령에 따른 상관분석은 Pearson correlation으로 실시하였다.
결과 : 20~40세 대상자의 등가구면 굴절력은 동공크기 3 mm에서 ?3.193±2.494 D, 5 mm에서 ?3.386±2.451D로 0.193 D 근시도가 증가하였으며, 45~80세 대상자의 등가구면 굴절력은 동공크기 3 mm에서 0.015±1.340D, 5 mm에서 ?0.054±1.566 D로 0.069 D 근시도가 증가하였다. 또한 동공크기 3 mm에서 20~40세 대상자보다 45~80세 대상자의 코마수차는 0.026 μm, 구면수차는 0.016 μm, 트레포일수차는 0.037 μm, 비점수차는 0.012 μm, 콰트레포일수차는 0.022 μm 증가하였다.
결론 : 노안교정을 위한 누진굴절력렌즈 처방 시 시력을 저하시킬 수 있는 동공크기나 고위수차를 참고하여 처방해야 할 것으로 생각된다.
Purpose : The purpose of this study was provide information that could be helpful in clinical practice for, presbyopia prescription as, classifying and analyzing refractive errors and highorder aberrations according to changes in pupil size into age groups before and after presbyopia.
Methods : Refractive errors and high-order aberrations were measured between 20~80 years of age adults males and females who had no eyes disease and refractive surgery, and they were divided into two groups such as, 20~40 years and 45~80 years. A paired-T test was used to compare refractive errors and high-order aberrations according to the changes in pupil size, and correlation analysis according to age was conducted by Pearson correlation.
Results : The spherical equivalent power of 20~40 years was ?3.193±2.494 D at 3 mm of pupil size, and the spherical equivalent power at 5 mm was ?3.386±2.451 D, showing that myopia of 0.193 D increased. The spherical equivalent power of 45~80 years was 0.015±1.340 D at 3 mm of pupil size, and the spherical equivalent power at 5 mm was ?0.054±1.566 D, showing that myopia of 0.069 D increased. In addition, at a pupil size of 3 mm, coma aberration of 0.026 μm, spherical aberration of 0.016 μm, trefoil aberration of 0.037 μm, astigmatism of 0.012 μm, and quatrefoil aberration of 0.022 μm was increased in 45~80 years more than those 20~40 years.
Conclusion : When prescribing progressive lenses for presbyopia correction, it is considered to be prescribed with reference to pupil size or high-order aberrations that can degrade vision.
