Abstract
정상균총이 병원성을 갖게되는 기회감염은 흔히 파종성 병변을 일으킨다. 급성 백혈병은
이러한 기회감염의 선행질환 중 가장 중요한 것의 하나이며, 진균성 기회감염의 70∼80%가
칸디다에 의한 것이기 때문에 급성 백혈병 환자에서는 칸디다 감염(칸디다증)의 중요성이
반드시 강조되어야 한다.
파종성 칸디다증에서는 피부, 심장, 뇌막, 간, 관절, 골수, 안구 등이 침범되어 염증성 병변
이 초래될 수 있음에도 불구하고 보고들에 의하면 사망전에 파종성 칸디다증을 진단할 수
있는 경우는 50%에 불과하다.
본 증례는 급성 림프구성 백혈병(acute lymphoblastic leukemia, ALL)환자에서 파종성 칸
디다증을 의심할 만한 소견이 없이 갑자기 각막 황달, 간비대 및 복부통증이 발생하여 ALL
이 간을 침범한 것으로 생각하였다가 사망후에 시행한 간침조직검사에서 우연히 발견된 간
의 칸디다증에 관한 것이다.

In view of the possible role of portal circulation in hematogenous spread of Candids
species, a case of hepatic candidiasis occurred in an eight-year-old child with acute
lymphoblastic leukemia (ALL) treated by chemotherapy is presented.
Symptoms and signs referable to the hepatic disease in this patient included
hepatomegaly, icteric sclera and abdominal pain. There were no particular manifestations
suggestive of deep mycotic involvement of any specific organs or tissues other than the
liver. Culture of the blood was negative for one month. On the 24th hospital day the
patient died with the presumptive diagnosis of ALL, disseminated intravascular
coagulation, acute renal failure, pulmonary edema, cholecystitis and oral thrush. A needle
necropsy was performed and revealed fungal aggregates replacing the large foci of
hepatic cell loss.
It is suggested that, when the gastrointestinal tract serves as the portal of entry, the
liver could be the visceral organ involved first in the course of disseminated candidiasis.
