Abstract
악성 림프종은 주로 림프절에서 발생하지만, 전체의 약 1/3정도가 위장관 등 림프절외 조
직에서 발생한다. 악성 림프종이 전신성 질환의 일부로서 담낭이나 간외담관 등을 침범하는
경우도 흔하지 않지만, 담낭에서 원발성으로 악성 림프종이 발생하는 경우는 아주 드물다.
이들 담낭의 악성 림프종은 대개 급성 담낭염의 증상으로 발현하며 증상없이 우연히 발견되
는 경우도 있다. 저자들은 담낭의 원발성 악성 림프종 1예를 경험하고 국내 첫번째 증례로
서 보고하고자 한다.

Lymphomatous involvement of the gallbladder is rarely identified and manifests mostly
as a local involvement of disseminated disease. There have been reported only about 20
primacy malignant lymphomas of the gallbladder without involvement of other sites.
A 63-year-old woman presented with jaundice, epigastric pain, and weight loss of two
months duration. There was no systemic lymphadenopathy elsewhere. Abdominal
computed tomograph resealed a focal thickening of the gallbladder wall. The removed
gallbladder contained a well circumscribed, 5×3 ㎝-sized, slightly elevated firm area
with accentuated mucosal papillary excrescences near the neck protion. The lesion
involved whole thickness of the wall, and was made up of diffuse atypical lymphoid cell
infiltrates of variable size accompanying plasmacytoid differentiation in areas. Some large
tumor cells were pleomorphic and contained prominent nucleoli. To our knowledge, this
is the first case of primary malignant lymphoma of the gallbladder in Korean literature.
